At the request of Kenny Williams, your installer, on October 14 and November 5, 2016, we visited
the construction site for your residence under construction on Lot 1, Siloam Springs PUD, 0780 Siloam
Springs Road, Dotsero, Eagle County, Colorado. The purpose of our site visits was to inspect the
installation of the septic system.
They installed the system in overall compliance with the septic system design, Drawing
No. 15-3037OWTS-Elim.dwg, last revised April 15, 2016, and as shown on the attached as -built
sketch. They installed a 1500-gallons, two compartments, plastic septic tank by Infiltrator and an
Eliminite Tanc 120C, also in a 1500-gallons, two compartments, plastic, septic tank by Infiltrator.
The septic tanks had three access openings on each. On each of the first and last openings, 18-inch,
plastic risers were constructed. A cleanout was constructed between the septic tank and the
building, as shown on the above -named as -built sketch..
Ninety-eight (98), Q-4, Plus Standard Low Profile Infiltrator Chambers were installed in
six (6) trenches. There were eighteen (18) infiltrators in the first trench, seventeen (17) in each of
the next four (4) trenches, and twelve (12) in the last trench. There was a minimum of six feet
separation between the trenches. In the first and last infiltrator of each trench, quick4plus, standard
all -in -one 8-inch end caps were installed. The inspection ports were not installed at the time of our
first site visit but were observed during the second site visit, after the leachfield was already
backfilled. A wooden post for the control panel was constructed west of the Eliminite tank. The
system was tested and put into operation by your installer, the Eliminite representative and
maintenance provider, Kenny Williams. He will be supplying a letter of the system start up.

On March 25'hof 2017 the Eliminite on site waste system located at 780 Siloam Springs Drive in Dotsero CO.
Was activated by Kenneth Williams From K.W.'S Pipefixation Who is Eliminite certified.
Upon startup of the Eliminite it was observed that the system consists of a SJE-Rhombus control panel,
Serial# I 39818X- 16223, one Drain field pump, one Eliminite pump both 120 volt, a 1500 Gal primary septic tank
And a 1500 Gal Eliminite tank. All components of the system functioned as designed.
