Install the OWTS exactly as depicted in the design report and drawings stamped, signed and dated
October 23, 2015 by Luiza Petrovska of LKP Engineering, Inc.
The design consists of installing a new 2,000 gallon Front range Precast two compartment septic tank
with access to grade and 1,800 square feet of soil treatment area (prior to adjustment for using chambers)
via the installation of 105 Infiltrator Quick 4 Plus chambers in 5 shallow, serially connected trenches with
each trench containing 21 chambers as shown on the design drawing. The system designed to serve a 6
bedroom residence will completely replace the original system installed in 1988 under ISDS 752.
Pay close attention to the setbacks and irrigation ditch encasements and crossings to assure compliance
with the design as well as the Eagle County Public Health Agency OWTS Regulations. The existing septic
tank must be abandoned in accordance with Regulation 43.9 (C) and documented as such to obtain final
approval for use of the system.
Do not install the trenches in wet or frozen ground and be sure to contact the design engineer to conduct
all inspections necessary to certify the system and to prepare the as-built drawings. System certification
along with as-built drawings, photos and documentation of proper tank abandonment are required to be
submitted to, and approved by, Eagle County Environmental Health prior to the use of the new system.
The new soil treatment area must be protected in perpetuity from surface irrigation and infiltration of
water from the pond and ditch system.

finalized.This permit was approved and sized for a 6 bedroom dwelling and will completely

Dear Lissa:
At the request of Dave Bronn, on November 4, and today November 10, 2015, we visited the Treat
property on Lot 8, Pilgrim Downs, Filing No. 5, 476 Pilgrim Drive, Eagle County, Colorado. The purpose of
our site visits was to inspect the installation of the septic system. This system is a replacement of the 1986
old system that had failed. The old leachfield will be abandoned in place, while the old septic tank will be
pumped out and taken to the Eagle County dump, according to the installer, Dave Bronn.
The new septic system was designed and built for a 6-bedroom residence. They installed the new
system in overall compliance with the septic system design, Drawing No. 15-3065OWTS.dwg, dated October
23, 2015, and as shown on the attached as-built sketch. At the time of the inspection we observed the
following:
They installed a 2000-gallons, two compartments, concrete, septic tank. The septic tank was by
Valley Precast Concrete. They constructed two cleanouts on the building sewer line near the house, for
troubleshooting in both directions, and one cleanout just before the building sewer line encasement. The
encasement consisted of a 6-inch diameter, SCH40 PVC and it was to extend at least ten feet on each side of
the piped irrigation ditch crossing. Insulation board will be placed over the building sewer line encasement
and a minimum of 18-inch separation will be maintained between the two pipes. We understand that the ditch
encasement will be done tomorrow and photographs will be provided.
For the leachfield, one hundred and five, Q-4 Plus, Standard Infiltrator Chambers were installed in
five trenches. There were twenty-one (21) infiltrators in each of the trenches. There was a minimum of six
feet separation between the trenches. In the first and last infiltrator of each trench, inspection ports were
installed. The soil exposed in the trenches was as observed in the profile hole. There is no other warranty
either expressed or implied.

On October 6, 8, 13, 14 and 19, 2015, we visited the property on Lot 8, Pilgrim Downs,
Filing No. 5, 476 Pilgrim Drive, Eagle County, Colorado. The purpose of our site visits was to
select the most suitable site available on the property that meets the criteria of OWTS #43, for a
replacement septic system for the existing residence plus one more bedroom for the future
buyer/owner. The site is a 5.11 acre parcel of land at 476 Pilgrim Drive, off of West Lake Creek
Road, off of Lake Creek Road, approximately 3.7 miles south from the intersection of Lake
Creek Road and Highway 6, Edwards, Eagle County, Colorado.
!Preliminary Investigation
G Address: 476 Pilgrim Drive, near Edwards, Eagle County, Colorado
G Legal Description: Lot 8, Pilgrim Downs, Filing No. 5, Eagle County,
Colorado
G Existing Structures: There is an existing residence on the lot.
G Location of existing and proposed wells on property: The existing well is
about 12 feet south of the southern most corner of the house and the three-
car garage.
G ECEHD records: According to the ECEH Department records, the
existing septic system consists of a 1250, gallons, septic tank and 825 sf
absorption area.
G Topography: The topography of the property is moderate. The existing
leachfield (which had failed and was observed to be full of water) was
located west of the residence. The new leachfield will be located on the
north side of the residence, parallel with the irrigation ditch, and down
gradient .
G Soil Data: The recommended percolation test for design was 30 mpi for
an application rate of 0.5 gpd/sf. The soil encountered in the profile hole
consisted:

G Location of applicable setbacks in Table 7-1: The proposed, new, septic tank is about 34
feet away from the house, 7 feet away from the leachfield, and about 10 feet away from
the lined irrigation ditch. The leachfield is at least 40 feet away from the house, and
about 160 feet away from the well.
!Reconnaissance
G Landscape position: The proposed leachfield site will be located on a
linear slope in the back yard of the house, about 13 feet away from the
lined/enclosed irrigation ditch.
G Topography: The topography of the proposed leachfield site is moderate
and the trenches will run along the contours.
G Vegetation: The vegetation of the proposed leachfield site consisted of
grass,.
G Natural and cultural features: The proposed leachfield site is presently
used as a hay field. There are several irrigation ditches running through
the property and the subdivision. The closest irrigation ditch is above the
leachfield and it will be lined or enclosed in a pipe. The second closest is
to the east and it is 50 feet plus away from the leachfield.
G Current and historic land use: Pilgrim Downs is private, gated
community, with residential lots. Previously the land was used for sheep
ranching.
!Detailed Soil Investigation
G Visual and tactile evaluation of two or more soil profile test pit
excavation; OR
G Percolation tests plus one or more soil profile holes or one or more profile
test pit excavations: Three profile holes were excavated in different areas
of the lot and the one logged was selected the most suitable. Also, three
percolation holes were dug, presoaked and a perc test was run the
following day.
