RESOLUTION NO. 88 -_�
AMENDMENT TO EAGLE COUNTY PERSONNEL POLICY
IMPLEMENTATION OF
DESIGNATED MEDICAL SERVICE PROVIDER PROGRAM
WHEREAS, pursuant to Section 30 -2 -104, C.R.S., and
Eagle County Resolution No. 87 -39, the Board of County Commis-
sioners of the County of Eagle, State of Colorado ( "Board "),
adopted a classification and compensation plan for all County
employees paid in whole or in part by the County, hereinafter
referred to as the 111987 Personnel Policy," as subsequently
amended pursuant to Resolution No. 87 -66; and
WHEREAS, the County of Eagle is a member of the County
Workers' Compensation Pool (the "Pool "); and
WHEREAS, in order to ensure better and more cost -
effective medical treatment for the employees of member counties
of the Pool, the Board of Directors of the Pool has required all
member counties to implement a designated medical service provider
program whereby any county employee suffering personal injury or
illness from a job- related accident shall be required to use a
county- designated medical service provider; and
WHEREAS, in order to comply with the Pool's require-
ments, the Board deems it appropriate to amend the 1987 Personnel
Policy with the designation of medical service providers and the
adoption of procedural guidelines to be followed by all Eagle
County personnel suffering from job - related injuries, as said
amendment is set forth on Exhibit A attached hereto and made a
part hereof by this reference.
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF THE COUNTY OF EAGLE, STATE OF COLORADO:
THAT, effective February 1, 1988, the Board adopts the
amendment to the 1987 Personnel Policy as said amendment is more
fully set forth on Exhibit A.
THAT, the 1987 Personnel Policy as herein amended is
hereby readopted.
THAT, the Eagle County Human Resources Director is
directed to notify all employees of the attached amendment prior
to its effective date of February 1, 1988.
THAT, the 1987 Personnel Policy, as herein amended,
shall apply to and shall be binding upon all County employees
paid in whole or in part by the County except as otherwise
provided for in Section 30 -2 -104, C.R.S., and as otherwise
provided for in the 1987 Personnel Policy.
THAT, should any section, clause, provision, sentence
or word of this Resolution, including the attached Exhibit A, be
declared by a court of competent jurisdiction to be invalid, such
decision shall not affect the validity of this Resolution as a
whole or the 1987 Personnel Policy as herein amended and re-
adopted, or any parts thereof, other than the part so declared to
be invalid. For this purpose, this Resolution is declared
severable.
THAT, the Board finds, determines and declares that
this Resolution is necessary for the health, safety and welfare
of the citizens of the County of Eagle.

We endorse the Eagle County Employee Handbook as hereby
amended, and support its applicability to all Eagle County
employees.

ACCIDENTS
Employees must advise their supervisor of any job related
accident that causes personal injury or property damage no
matter how minor. In the event of an on- the -job injury or
illness the following procedure is to be followed by employees:
A. Employees are to notify their supervisor
immediately of an on- the -job injury no
later than the end of the shift on which
the injury occurred.
B. Employees who incur a job - related injury
or illness which requires medical treatment
are required to report to the county desig-
nated provider listed below.
C. If the injury requires immediate emergency
treatment, utilize the nearest emergency
facility. The employee must then report the
injury to the designated provider.
D. The supervisor must complete the Employer's
First Report of Injury form. If possible,
the employee should also sign the form. The
form must be submitted to the Human Resources
Department within 24 hours of the accident.
Failure to comply with utilization of the designated provider
will result in a denial of payment for medical expenses through
the County Worker's Compensation Pool.
