If applying for partial reimbursement on behalf of processors or end users for raw waste tires located at a state, county or municipal owned and operated or approved storage or disposal facility, please answer Questions 11-16. 1. Describe the waste tire cleanup project to be conducted under this grant program and describe why the waste tires to be removed are illegally disposed or stored in their current location. Cite applicable regulations and ordinances. 2. Where are the illegal tires currently located? (Please provide a map). 3. What is the estimated number (or weight) of tires to be removed at each site? 1 (Rev. 7/01) 4. Where and how will the illegal tires be transported? 5. Will the tires be recycled, reused or land disposed? If disposed in a licensed landfill, explain' why the tires cannot be recycled or reused. 6. What is the estimated cost to remove and transport the waste tires? Please detail labor, equipment and administration and any other related costs? (Attach additional sheets if necessary). 7. How were the costs derived? Please note that a competitive bidding process is required unless county or municipal labor and equipment is used to implement the entire project. 8. Eagle County Landfill located at 2250 Highway 131, Wolcott, CO 81655 (2 miles north of Wolcott and 1.5 miles east of State Highway 131) 12. How many tons or tires of raw waste tire material will be processed at the facility? Approximately 650 tons or 65,000 scrap tires will be processed. 13. How will the processed tire material be used? Scrap tire chips will be utilized as the leachate drainage laver material for Waste Disposal Module 4 of the Eagle County Landfill. Eagle County Landfill was approved by the Colorado Department of Public Health & Environment on March 24, 2000 to incorporate this modification into the Engineering Design and Operations Plan for this facility. The 2 (Rev. 7/01) Jr modified design will utilize a minimum ten (10) inch thick laver of tire chips for the drainage laver of the leachate collection system. 14. What is the per ton cost to process or use the raw waste tire material? (Note: Reimbursement to processors and/or end users cannot exceed $50 per ton). The per ton cost to process the raw waste tire material is $110 per ton (please see attached_ contract). 15. Estimated Project Start Date: May, 2002 16.
