This Contract Change Order Letter is issued pursuant to paragraph C. of the Contract with contract routing number 01 FFA 00003 and
contract encumbrance number PO FFA CPD01000003A. This contract change order letter is between the DEPARTMENT OF PUBLIC
HEALTH AND ENVIRONMENT and the BOARD OF COUNTY COMMISSIONERS OF EAGLE COUNTY for the use and
benefit of the Eagle County Environmental Health Department. The Contract has been amended by Contract Renewal Letter 1
contract routing number 02 FFA 00003. The Contract, as amended, if applicable, is referred to as the "Original Contract" A copy of the

Enclosed please find an original, fully executed contract change order letter reducing your 2001-02
environmental health contract. This is for your files and requires no action on your part. As you are
aware, all environmental health county contracts were reduced by 1.6% to meet the mandatory
general fund reduction. To reiterate, this is a change to the current year's contract and does not affect
your 2002-03 contract. To implement this reduction, your 2001-02 fourth quarter payment will be
reduced by the amount listed on your change order letter. If you have any questions, please contact
