In accordance with Paragraph C.5 of the contract with routing number 9900499 and contract encumberance number PO FAA
EPS9900499, hereinafter referred to as "the Original Contract" (a copy of which is attached hereto and by this reference
incorporated herein and made a part hereof) between the State of Colorado, Department of Public Health and Environment
(Family and Community Health Services Division) and The Board of County Commissioners, Eagle County . covering the
period from July 1, 1998 through June 0 1999, the parties agree that the maximum amount payable by the State for the
eligible services in Paragraph C.1. of the Original Contract is decreased by ONE THOUSAND SIX HUNDRED SIXTY
EIGHT AND N0/100 DOLLARS ($ 6 8 0 for a new total of T1N THOUSAND ONE HUNDRED AND FIFTY AND
NO/100 DOLLARS (S 10.150.00). This fimding change is in accordance with the revised budget, which is incorporated
herein by this reference, made a part hereof, and attached hereto as Attachment A. The -first -sentence in Paragraph C.1. of the
Original Contract is hereby modified accordingly. All other terms of conditions of the Original Contract are hereby
reaffirmed by the parties.
This amendment to the Original Contract is intended to be effective as of A- Aril 15- 1999, but in no event shall it be deemed
valid until it shall have been approved bathe State Controller or such assistant as he may ry desi ng,�ate.
