Addendum to the Contract
between the Eagle County Department of Health & Human Services and Jose Banuelos
On the 23rd day of November, 1998, Eagle County Health & Human Services entered into a
contract with Jose Banuelos, an independent contractor, for the purchase of individual and family
treatment services under the County's CORE Services and WRAP Services programs for
children and families. The maximum amount of that contract for the period of August 1, 1998
through May 30, 1999 was $6,000.
Based on current service needs and utilization of these purchased services, it now anticipated that
the contracted need for services is in the amount of $10,000. Therefore the County and the
Contractor wish to amend the original contract to a maximum amount of $10,000 for the period
of August 1, 1998 through May 30, 1999.
