AGREEMENT
TO RENEW
JAIL MEDICAL CARE AND HEALTH SERVICES AGREEMENT

THIS AGREEMENT is entered into tais IBC %h day of R
i999 by and between the Board of County Commissioners of the County
of Eagle, State of Colorado and the Eagle County Sheriff,
hereinafter referred to jointly as "Detention Facility" and Michael
L. Beals, D.O., hereinafter referred to as "Contractor" to renew
the Agreement dated September 8, 1994, ("Original Agreement")
whereby Contractor will provide medical services to the Detention
Facility that complies with the Standards for the Accreditation of
Medical Care and Health Services in Jails, Eagle County's Detention
Health Administration Standards and the Eagle County Jail Health
Care Services Agreement ("Standards") and other applicable local,
state and federal law.
The parties agree that said Agreement shall be effective from
December 31, 1998 to December 31, 1999 and will be renewed
annually, unless terminated by either party according to the terms
of the Original Agreement.
The consideration for this Agreement shall be at the rate of
One Hundred Eighty and No/100 ($180.00) per hour.
Contractor shall provide an updated Certificate of Insurance
to the County each year said Agreement is in effect.
All other provisions of the Original Agreement shall be in
full force and effect.
