Contract Renewal Letter Number 1, Contract Routing Number 03 FAA 00173
State Fiscal Year 2002 - 01 Public Health Nursing Services Program
This Contract Renewal Letter is issued pursuant to paragraph D4. of the Contract with contract routing number 02 FAA.00063
and contract encumbrance number PO FAA OLL0200063. This Contract Renewal Letter is between the DEPARTMENT OF
PUBLIC HEALTH AND ENVIRONMENT and the BOARD OF COUNTY COMMISSIONERS OF EAGLE COUNTY (a
political subdivision of the state of Colorado), for the use and benefit of the Eagle County Public Health Nursine Service.
A copy of the Original Contract is incorporated herein by this reference, made a part hereof and attached
hereto unless a Letter of Compliance has been submitted with this Contract Renewal Letter. This Contract Renewal Letter is for
the new term of July 1. 2002 through June 30, 2003. The maximum amount payable by the State for the work to be performed
by the Contractor during this new term is increased/deereased by $58,471.48 for an amended total financial obligation of the
State of $115 620.98.
"4ttaehment 4 " The first sentence in paragraph D. of the Original Contract is modified accordingly. All other terms and
conditions of the Original Contract are reaffirmed. The proposed effective date of this Contract Renewal Letter is July 1, 2002.
However, in accordance with section 24-30-202, C.R.S., as amended, in no event shall this Contract Renewal Letter be deemed
valid until it shall have been aooroved by the State Controller or such assistant as he may designate.
Please sign, date, and return all 4 originals of this Contract Renewal Letter by June 24, 2002, to the attention of. Steve
