This Contract Renewal Letter is issued pursuant to paragraph CA. of the Contract with contract routing number of FAA
00759 and contract encumbrance number FAA PRN01-00759 between the COLORADO DEPARTMENT OF PUBLIC
HEALTH AND ENVIRONMENT and BOARD OF COUNTY COMMISIONERS OF EAGLE COUNTY a political.
subdivision of the state of Colorado. This Contract has been amended by Contact Renewal Letter #1 contract routing '

prenatal servicert program, purchase medical supplies and pharmaceuticals -and to provide medical, laboratory, education and -
counseling services ,to low income women, and/or Forty -Five 4Thousslad Dollars ($45,000.00) is for diagnostic: tests. The
number of patients to be served is approximately 150 patients.:
The first sentence in paragraph C.I. of the Original Contract is modified accordingly. All other terms and conditions of the
Original Contract are reaffirmed. The proposed effective date of this.Contract Renewal Letter is October 1, 2002. However,'
in accordance with section 24-30-202, C.R.S., as amended, in no event shall this Contract Renewal Letter be deemed valid
until it shall have been approved by the State Controller or such assistant as he may designate.
Please sign, date, and return all 4 originals of the Contract Renewal Letter by August 15, 2002, to the attention of: Laura
