HomeMy WebLinkAboutC22-357 MOU_Aspen Valley HospitalMemorandum of Understanding Between Eagle County And Aspen Valley Hospital This Memorandum of Understanding (“MOU”) sets forth the terms and understanding between Eagle County (“County”) and Aspen Valley Hospital (“AVH”) to recruit patients of the Aspen Birth Center at Aspen Valley Hospital who are Eagle County residents into the Family Connects program. Background Family Connects is a nurse home visiting program available to Eagle County residents voluntarily and at no cost. Family Connects is an evidence-based model that combines connection to community resources with short-term nurse home visiting in the first month after birth. Eagle County Public Health and Environment (“ECPHE”) is launching the Family Connects program in the fall of 2022 to all Eagle County residents. Recruitment of the program typically happens bedside at the hospital prior to patient discharge after giving birth. Purpose This MOU will establish a relationship between AVH and ECPHE with the focus of recruitment of Eagle County residents into the Family Connects program. This will be done through sharing Family Connects information with Eagle County patients at the Aspen Birth Center at AVH. The above goals will be accomplished by undertaking the following activities: Responsibilities and Obligations of the Parties ● AVH will ○ Add a question to discharge paperwork about whether or not a patient is an Eagle County resident ○ Provide information to patients who are Eagle County residents about the Family Connects program and make them aware of the fact that they are eligible to participate in Family Connects at no cost         DocuSign Envelope ID: 616CA24F-7703-481D-891D-B7EEDA62766F ○ Provide the “Consent to Contact” form to patients who are Eagle County residents, allowing them to opt-in to being contacted by an ECPHE employee regarding the Family Connects program ○ Share the signed “Consent to Contact” forms with ECPHE via email every two weeks so that we can follow up with patients in a timely manner ○ In special circumstances, provide access to patients for recruitment to Family Connects via floor access ○ Provide program marketing materials to all labor and delivery patients ○ Provide monthly de-identified birth data for Eagle County residents ● ECPHE will ○ Provide the Family Connects services to Eagle County families who deliver at AVH and consent to participate in the Family Connects program ○ Provide training to hospital nurses on key talking points regarding the Family Connects program ○ Provide feedback regarding trends in data regarding AVH patients ○ Communicate with AVH’s providers as necessary ○ Provide Family Connects marketing materials to AVH staff and providers Funding This MOU is not a commitment of funds by either party. Duration This MOU is at-will and may be modified by mutual consent of authorized officials from County and AVH. This MOU shall become effective upon signature by the authorized officials from County and AVH and will remain in effect until modified or terminated by any one of the partners by mutual consent.
