Tax Parcel # 2105-302-01-001. Property Location: 2428 Casteel Creek Rd.,
Edwards, CO., Levine residence.
Enclosed is your ISDS Repair Permit No. 2130R-01, and a copy of the engineer design revision
letter stipulated on the permit. Please obtain a copy of the original design from the engineer. It
is valid for 120 days, or for the duration of your current building permit for this property. The
enclosed copy of the permit must be posted at the installation site. Any changes in plans or
specifications invalidates the permit unless otherwise approved.
Systems designed by a Registered Professional Engineer must be certified by the Engineer
indicating that the system was installed as specified. Eagle County Environmental Health
must also view the installation prior to back filling. Please call well in advance for your
final inspection. Your TCO will not be issued until our office receives this certification.
Permit specifications are minimum requirements only, and should be brought to the property
owner's attention.
This permit does not indicate conformance with other Eagle County requirements.

Dear Mr. Levine,
This letter is to inform you that the above -referenced ISDS Permit has been inspected and
finalized. Your septic system has been sized to accommodate an 8 bedroom residence. Enclosed
is a copy to retain for your records. Additional information about the maintenance of your septic
system as well as information about private wells can be accessed through web site links provided
on the Environmental Health Department's home page at Imttp://www.eaglecounty.us/envHealth/
Be aware that changes in the use of your property or alterations of your building may
require commensurate changes to, or relocation of, your septic system. Landscape features,
trees with tap roots, irrigation systems and parking areas above the drain field can cause
premature system failure. It is equally important that you notice and immediately repair
dripping faucets and hissing toilets as this will certainly cause the system to fail. By
following a few simple guidelines, like avoiding grease or other additives down the drain and
making sure your septic tank is on a pumping schedule, your system has been designed to
last for many years.

At the request from Marc Trudeau with Thomas and Warner Construction, on August 9,
13, and 15, 2001, we visited the construction site at Casteel Creek Ranch, The purpose of our site
visit was to observe the leachfield for the main residence.
It appears that the leachfield that was completed and inspected on August 26, 1998, had
been damaged by construction activities. Steve Jewett -had- the ends of all the infiltrator trenches
open for an -inspection. We found that due -to heavy traffic, over the leachfield,-the infiltrators
have been pushed into the ground and'the infiltrative area of the leachfield has been damaged and
reduced. The backfill over the leachfield 'is greater than the recommended three feet, and the
ground surface over the leachfield is very irregular with many low spots that can accumulate
surface runoff and overload the system. Also, water from the main residence, from drains- and
uses other than the ones included in the design capacity of the system (roof drains, terrace, deck,
etc..,) have been draining into the leachfield.
Based on the condition of the leachfield, we recommend that a new leachfield be
constructed. Five, new trenches should be excavated between the existing trenches with
infiltrators. The distance between the existing trenches with infiltrators is I I feet center to center,
which will allow to safely construct five new trenches into undisturbed soil. These trenches
should be 24 inches deep. The -bottom of the trenches should be raked. Do not construct the new
trenches in wet soil. Eighty, ne infiltrator camber should be constructed in the new trenches,
while the old ones are abandoned. The infiltrators will be connected with serial distribution.
Inspection ports should be constructed in the first and last infiltrator of each trench.
New type of discharge should be provided for the drainage needs from the main
residence, which are not part of the design criteria for the septic system. The grade above the
leachfield should be such as to provide rapid runoff away froze the leachfield, and not accumulate
on top of the leachfield.

At the request of Mr, Curt McDonald, the superintendent from Structural Associates, on August
26 and October 21, and November 27, 1998, we visited the construction site on Lot 2, Casteel Creek
Ranch, Eagle County, Colorado. The purpose of our site visits was to observe the installation of the septic
system.
They installed the system in overall compliance with the septic system design, Drawing No.
96102SL),DWG, last revised August 5, 1998.
The system was connected to the main residence as shown on the above named drawing. They
installed 2500-gallon, two compartments, a precast, concrete septic tank. From the tank to the leachfield
was about 1200 feet, with cleanouts every 100 feet. As -built drawings by the installer Logan Craig, Inc-,
showing the layout and the cleanouts between the buildings and the septic tank, and from the septic tank
to the leachfield is included with this letter,
Eighty Standard Infiltrator Chambers were installed in five trenches with 16 Infiltrators each. The
infiltrators were connected with serial distribution. In the last infiltrator of each trench, inspection ports
were installed. Horizontal distance from center to center of the trenches, was I I feet or more. All piping
was SDR-35, 4-inch PVC, The depth of the trenches was three feet. The soil profile in the trenches was
consistent with the soil exposed in the profile hole.

At the, request from Bart Thomas with Thomas and Warner Construction, on September
18, 2001, we met at the construction site for the main residence, at Casteel Creek' Ranch. The
purpose 01 our meeting,,vas to evaluate the interior sources of wastewater connected to the
Jeachfield for the main residence.
Shane was working on an as-bu.ilt drawing of all the interior and exterior sources of
wastewater connected to the leachficld for the main. house. During a telepb.one conversation with
Shane (this week), he informed us that he has completed his report and copies were forwarded to
your office. IVe have not received a copy of that report.
As discussed during the September 18"' meeting, I talked with Mr. Ray Marry of the.
Eagle County EnviTonrnental Health Department, about the allowed and disallowed sources of
wastewater for the leachfield, and they are:
Allowed sources of wastewater f Ton.), residential uses: dishwasher, washing
machine, shower/bath, toilet, kitchen sink, lavatories, and floor drains from a
mechanical room,
Disallowed sources of wastewater: pool discharge, water treatment system, garage
drain, roof drains, patio d3.ains, etc... (And any wastewater that will -increase the,
maximum design flow or deposit unknown substances that may affect the function
and durability of the systern.)
Garage drains should have an oil and sand trap from where the water earl discharge to A
daylight location. Drains from patios and roof drains can discharge by gravity to a daylight
location -away from. the limits of back -fill for the foundation.
It is recommended that the wastewater from the pool and.from the water treatment system
discharge to a lined detentim) pond with a spillway, where chemicals and minerals can settle
down and the water can overflow overland.

Dear Kyle:
At the request from Marc Trudeau with Thomas and Warner Construction. on August 9,
13, and 15, 2001, we visited the construction site at Casteel Creek Ranch. The purpose of our site
visit was to observe the leachfield for the main residence.
It appears that the leachfield that was completed and inspected on August 26, 1998, had
been damaged by construction activities. Steve Jewett had the ends of all the infiltrator trenches
open for an inspection. We found that due to heavy traffic over the leachfield, the infiltrators
have been pushed into the ground and the infiltrative area of the leachfield has been damaged and
reduced. The backfill over the leachfield is greater than the recommended three feet, and the
ground surface over the leachfield is very irregular with many low spots that can accumulate
surface runoff and overload the system. Also, water from the main residence, from drains and
uses other than the one"; included in the design capacity of the system (roof drains, terrace, deck,
etc..,) have been draining into the leachfield.
Based on the condition of the leachfield, we recommend that a new leachfield be
constructed. Five., new trenches should be excavated between the existing trenches with
infiltrators. The distance between the existing trenches with infiltrators is 11 feet center to center,
which will allow to safely construct five new trenches into undisturbed soil. These trenches
should be 24 inches deep. The bottom of the trenches should be raked. Do not construct the new
trenches in wet soil. Eighty, ne infiltrator camber should be constructed in the new trenches,
,while the old ones are abandoned. The infiltrators will be connected with serial distribution.
Inspection ports should be constructed in the first and last infiltrator of each trench.
New type of discharge should be provided for the drainage needs from the main
residence, which are not part of the design criteria for the septic system. The grade above the
leachfield should be such as to provide rapid runoff away from the leachfield, and not accumulate
on top of the leachfield.

radius is 16 feet. For a square shaped pond. 2 feet deep, the sides will be 28 by 28 feet.

Dear Mr. Levine:
At the request of Mr. Curt McDonald, the superintendent from Structural Associates, on August
26 and October 21, and November 27, 1998, we visited the construction site on Lot 2, Casteel Creek
Ranch, Eagle County, Colorado. The purpose of our site visits was to observe the installation of the septic
system.
They installed the system in overall compliance with the septic system design, Drawing No.
961025D.DWG, last revised August 5, 1998.
The system was connected to the main residence as shown on the above named drawing. They
installed 2500-gallon, two compartments, a precast, concrete septic tank. From the tank to the leachfield
was about 1200 feet, with cleanouts every 100 feet. As -built drawings by the installer Logan Craig, Inc.,
showing the layout and the cleanouts bet veep the buildings and the septic tank, and from the septic tank
to the leachfield is included with this letter.
Eighty Standard Infiltrator Chambers were installed in five trenches with 16 Infiltrators each. The
infiltrators were connected with serial distribution. In the last infiltrator of each trench, inspection ports
were installed. Horizontal distance from center to center of the trenches, was 11 feet or more. All piping
was SDR-35, 4-inch PVC. The depth of the trenches was three feet. The soil profile in the trenches was
consistent with the soil exposed in the profile hole.
