Enclosed is your ISDS Alteration Permit No. 2066A-00, along with a copy of the engineer's
design stipulated on the permit. It is valid for 120 days. The enclosed copy of the permit must
be posted at the installation site. Any changes in plans or specifications invalidates the
permit unless otherwise approved. Please note any special requirements that may have
been added to the design by this department.
Systems designed by a Registered Professional Engineer must be certified by the Engineer
indicating that the system was installed as specified. Eagle County does not perform final
inspections on engineer designed systems. Your TCO will not be issued until our office
receives this certification.
Permit specifications are minimum requirements only, and should be brought to the property
owner's attention.
This permit does not indicate conformance with other Eagle County requirements. Please
notify this office if you have not been contracted to perform this installation.

At the request of Dave Stanish and Rick Filters3 on .tune 8, July 19, August 8,10, and 1. 7
2001, we visited the constriction site at 0909 the Forest, Eagle County, Colorado. The purpose
of our site visits was to observe the installation of the septic system. In the process revisions were
made to the design of the leachfield part of the system -
They installed the system in overall compliance with the septic system design, Drawing
No. 99248SDPUMP.DWG, last revised August 13, 2001, as shown on the attached as -built
sketch. They installed a 1000-gallon, three compartments, a precast, concrete septic tank_ In the
last compartment of the septic tank, was a Zoeller Pump, Model 282. The tm* had three access
openings, one over each of the compartments. The effluent line from the pump tank to the first
infiltrator of the leachfield was a total distance of 25 feet. The effluent line was 3-inch diameter
sch40 PVC. The pump was sized to have 225 gallons per dose. The dosing, the floats function
and the alarms were tested by Davis Excavating. Davis Excavating also noted the location of the
alarm system to be in the lower level of the mechanical, room on the southwest wall.
Forty-eight, Standard Infiltrator Chambers were installed in three trenches. Each row of
trenches contained sixteen infiltrators. The infiltrators were connected with serial distribution_ In
the first azid last infiltrator of each trench, inspection ports were installed. In addition, in the
second row of infiltrators an inspection port was installed in the eighth infiltrator from the south.
Horizontal distance from oonter to center of the trenches, was 11 feet or more. The depth of the
trenches was no more than 36 inches.
