EAGLE COUNTY, COLORADO
AND
AUZIMI CONSULTING, LLC
THIS FOURTH AMENDMENT (“Fourth Amendment”) is effective as of __________________
by and between Auzimi Consulting, LLC a limited liability company (hereinafter “Consultant”
or “Contractor”) and Eagle County, Colorado, a body corporate and politic (hereinafter
“County”).
RECITALS
WHEREAS, County and Consultant entered into an agreement dated the 2nd day of February,
2020, for certain Services (the “Original Agreement” C20-387); and
WHEREAS, County and Consultant entered into an agreement dated the 28th day of October,
2020 for certain Services (the “First Amendment” C20-394); and
WHEREAS, County and Consultant entered into an agreement dated the 1st day of January,
2021 for certain Services (the “Second Amendment” C20-397); and
WHEREAS, County and Consultant entered into an agreement dated the 14th day of June, 2021
for certain Services (the “Third Amendment” C21-211); and
WHEREAS, the County desires to have Consultant perform additional Services for additional
compensation as set forth below.
FOURTH AMENDMENT
NOW THEREFORE, in consideration of the foregoing and the mutual rights and obligations as
set forth below, the parties agree as follows:
1.The Third Agreement shall be amended to include additional Services as described in
Exhibit A-4, which is attached hereto and incorporated herein by reference.
2. The compensation for the additional Services set forth in Exhibit A-4 shall not exceed
$27,000 or a total maximum compensation under the Original Agreement, the First
Amendment and the Second Amendment of $100,875.
3.Capitalized terms in this Fourth Amendment will have the same meaning as in the
Original Agreement, the First Amendment, the Second Amendment, and the Third
Amendment. To the extent that the terms and provisions of the Fourth Amendment
conflict with, modify or supplement portions of the Original Agreement, the First
Amendment, the Second Amendment, and the Third Amendment, the terms and
provisions contained in this Fourth Amendment shall govern and control the rights
and obligations of the parties.

4.Except as expressly altered, modified and changed in this Fourth Amendment, all
terms and provisions of the Original Agreement shall remain in full force and effect,
and are hereby ratified and confirmed in all respects as of the date hereof.
5.This Fourth Amendment shall be binding on the parties hereto, their heirs, executors,
successors, and assigns.
IN WITNESS WHEREOF, the parties hereto have executed this Fourth Amendment to the
Original Agreement the day and year first above written.

SCOPE OF SERVICES, SCHEDULE, FEES
1.PURPOSE:
Eagle County Department of Human Services (ECDHS) provides a wide array of State &
Federally mandated services to ensure the safety and stability of the most vulnerable
populations in Eagle County. ECDHS seeks to contract with a professional to provide services
outlined below.
2.CONSULTANT RESPONSIBILITIES:
Contractor will continue facilitating program design related to the ECDHS child abuse prevention
projects, including the Child Maltreatment Prevention Plan and informal child care provider
engagement and support planning.
●Child Maltreatment Prevention Plan
○Facilitate Family Wellbeing Collaborative (FWA) meetings/workshops
○Complete full update to original FWC data & analytics
○Execute CMPP ‘Goal: Prescriptive Feedback Loops’
○Develop and gain consensus on FWC Charter and expand membership including
‘voice of family’ reps
○ID and prioritize Quick Hits
○Additional content creation: Meetings, marketing, educational
●Family Friends and Neighbors (FFN) Engagement and Support Blueprint
○Phased plan with prioritized activities and goals established for second half of the
Colorado Health Foundation grant Year 2, full Year 3 of grant.
○Comprehensive framework for building trust between government and the FFN
community
○Attributes of Eagle River Valley’s FFN community quantified
○Preliminary FFN engagement (sample population)
○Survey plan and instrument(s) developed for Voice of FFN
○FFN communications channels preferences understood
○Best fit supports/resources self identified by FFNs
3.FEES AND COMPENSATION:
County shall pay Contractor as follows:
●Monthly payments of $9,000 per month for three months, beginning upon execution of
contract amendment.
●Total of additional scope compensation not to exceed $27,000
