EAGLE COUNTY, COLORADO
AND
AUZIMI CONSULTING, LLC
THIS THIRD AMENDMENT (“Third Amendment”) is effective as of __________________ by
and between Auzimi Consulting, LLC a limited liability company (hereinafter “Consultant” or
“Contractor”) and Eagle County, Colorado, a body corporate and politic (hereinafter “County”).
RECITALS
WHEREAS, County and Consultant entered into an agreement dated the 2nd day of February,
2020, for certain Services (the “Original Agreement” C20-387); and
WHEREAS, County and Consultant entered into an agreement dated the 28th day of October,
2020 for certain Services (the “First Amendment” C20-394); and
WHEREAS, County and Consultant entered into an agreement dated the 1st day of January,
2021 for certain Services (the “Second Amendment” C20-397); and
WHEREAS, the County desires to have Consultant perform additional Services for additional
compensation and extend the timeframe of the contract as set forth below.
THIRD AMENDMENT
NOW THEREFORE, in consideration of the foregoing and the mutual rights and obligations as
set forth below, the parties agree as follows:
1.The Second Agreement shall be amended to include additional Services as described
in Exhibit A-3, which is attached hereto and incorporated herein by reference.
2. The compensation for the additional Services set forth in Exhibit A-3 shall not exceed
$18,875 or a total maximum compensation under the Original Agreement, the First
Amendment and the Second Amendment of $73,875.
3. The term of the contract shall be extended for 12 months from the effective date
stated above.
4.Capitalized terms in this Third Amendment will have the same meaning as in the
Original Agreement, the First Amendment and the Second Amendment. To the extent
that the terms and provisions of the ThirdAmendment conflict with, modify or
supplement portions of the Original Agreement, the First Amendment and the Second
Amendment, the terms and provisions contained in this Third Amendment shall
govern and control the rights and obligations of the parties.

5. Except as expressly altered, modified and changed in this Third Amendment, all
terms and provisions of the Original Agreement shall remain in full force and effect,
and are hereby ratified and confirmed in all respects as of the date hereof.
6. This Third Amendment shall be binding on the parties hereto, their heirs, executors,
successors, and assigns.
IN WITNESS WHEREOF, the parties hereto have executed this Third Amendment to the
Original Agreement the day and year first above written.

Exhibit A-3
SCOPE OF SERVICES, SCHEDULE, FEES
1. PURPOSE:
Eagle County Department of Human Services (ECDHS) provides a wide array of State &
Federally mandated services to ensure the safety and stability of the most vulnerable
populations in Eagle County. ECDHS seeks to contract with a professional to provide services
outlined below.
2. PROVIDER RESPONSIBILITIES:
Contractor will continue facilitating program design related to the ECDHS child abuse prevention
projects, including the Child Maltreatment Prevention Plan and the previously called Rewiring
initiative and begin project management planning for customer feedback loops for all ECDHS
programming.
● Child Maltreatment Prevention Plan
○ This work focuses on identifying preventative strategies to protect children in
Eagle County from abuse and neglect.
○ Coordination of family voice inputs via survey and focus groups. Please note the
contractor is not expected to facilitate the focus groups but will plan and prepare
ECDHS staff to facilitate said focus groups.
○ Submission of Eagle County Child Maltreatment Prevention Plan Action Plan
using template from Colorado Department of Human Services Office of Early
Childhood and a local version of the plan for community distribution and other
associated tasks.
● Previously called “Rewiring” child abuse prevention initative
○ Facilitation roles will include workgroup meeting preparation and correspondence
○ Measurement, data collection and analysis.
● ECDHS Customer Feedback Loops
○ project management and communications related to planning, research,
development, testing, communication and deployment of customer feedback
collection loops.
3. FEES AND COMPENSATION:
County shall pay Contractor as follows:
● Payment of $6,187.50 for the completion of collecting family voice (anticipated by August
31, 2021)
● Payment of $6,187.50 for the completion of the Child Maltreatment Prevention Plan
Action Plan - including state template and local version (anticipated by October 29,
2021)
● Payment of $6,500 for the completion of the Customer feedback loop(s) work
(anticipated by November 30, 2021)
● Total of additional scope compensation not to exceed $18,875
