MEMORANDUM OF UNDERSTANDING FOR MIRA PROJECT IMPROVEMENT PLAN
This Memorandum of Understanding (MOU) entered on ________________ creates an
agreement between EAGLE VALLEY COMMUNITY FOUNDATION, VAIL HEALTH, and EAGLE
COUNTY GOVERNMENT to implement the MIRA PROJECT IMPROVEMENT PLAN.
The agreed upon Purpose of this improvement plan are:
1.Improve MIRA Wi-Fi accessibility. Being equipped with consistent digital access can affect resources
that are provided on MIRA. From social supports to civic agency, tech access can deliver services
that are intrinsically tied to collective health and equity outcomes. Improved digital access also
allows for MARTI translation services through Vail Health to be consistently available, broadening
opportunities for service providers to extend mobile access.
Scope of Work: Acquire the necessary equipment and labor for installation to enhance cellular data
throughout the mobile range for MIRA’S scheduled destination.
Responsibility & Timeframe: This work will commence as soon as Melina Valsecia can arrange
scheduling with Eagle County Department of Technology.
Eagle Valley Community Foundation agrees to pay for work within 10 days of invoice through funds
provided through Vail Health. Cost: $2,500
Monthly service charges will be administered and paid by Eagle Valley Community Foundation.
Annual cost: $600
Anticipated date of completion: October 31, 2020
2.Reduce Health Disparities. Extending health coverage to more low-income people is an important
way to reduce health disparities among people of color. Director of Acute Care and Population
Health of Colorado Mountain Medical has strongly encouraged increasing Medicaid/Medicare
enrollment. MIRA will provide a position for a Medicaid/ Medicare enrollment specialist, trained by
Colorado Department of Human Services and Connect for Health Colorado.
Scope of Work: The Enrollment Specialist will work approximately 6 hours per week, supervised by
MIRA Supervisor. A laptop will be provided to the enrollment specialist. Cost: $6,400
Responsibility & Timeframe: Ongoing position with quarterly monitoring of Medicaid/Medicare
enrollment. Hiring and employee set-up will be arranged by Melina Valsecia within 30 days of MOU.
Payroll requirements will be administered and paid by Eagle Valley Community Foundation, through
funds provided for one year from Vail Health.
Payment: Vail Health agrees to provide funding directly to Eagle Valley Community Foundation in
the amount of $9,500. Funding will cover expenses related to #1. Wi-Fi accessibility on MIRA, and
#2. Expenses for Medicaid/Medicare enrollment specialist for one year.

that the signatories shall strive to reach, to the best of their abilities, the purpose stated in this MOU.
