BOARD OF COUNTY COMMISSIONERS
COUNTY OF EAGLE, STATE OF COLORADO
RESOLUTION NO. 92 -
CONCERNING THE DESIGNATION OF EAGLE COUNTY
IN A LONG TERM CARE SINGLE ENTRY POINT SYSTEM
DEPARTMENT OF SOCIAL SERVICES
WHEREAS, the rules of the Colorado Department of Social
Services ( "State Department "), Volume 8.020, "Long Term Care Single
Entry Point System," mandate that the boards of county commis-
sioners of each county in the State of Colorado submit to the State
Department, no later than March 31, 1992, their recommendations for
designation in a single entry district, according to the following
guidelines:
1. The combined boards of county commissioners shall submit
their recommendation to the State Department.
2. The recommendation submitted by the county commissioners
shall include the following information:
a. The geographic boundaries.
b. The organizational structure, operational methods,
and decision - making process of the boards of county
commissioners.
C. The boards of county commissioners will assure that:
(1) All counties of the area proposed are involved
in the district.
(2) All counties are contiguous.
(3) One full -time case manager will be employed by
the single entry point agency.

(4) There will be adequate staffing by the single
entry point agency to provide coverage for all case
management functions and administrative support in
accordance with Volume 8.023.0 of the rules of the
State Department.
3. The State Department shall approve the recommendation of
the district no later than April 30, 1992.
NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF COUNTY
COMMISSIONERS OF THE COUNTY OF EAGLE, STATE OF COLORADO:
THAT, Eagle County join with the Counties of Garfield, Pitkin,
Rio Blanco, Moffat, and Routt (the "Member Counties "), to form a
Single Entry Point District ( "District ") for the purpose of
enabling persons in need of long -term care to access the services
of the State Department, pursuant C.R.S. 26 -4 -522.
THAT, the Garfield County Department of Social Services shall
act as the Single Entry Point Agency, with a policy board composed
of a representative from each member county.
THAT, Eagle County join the other Member Counties in providing
the following assurances:
1. The Member Counties are contiguous.
2. The District shall employ at least one full -time case
manager.
3. The District will be adequately staffed to provide
coverage for all case management administrative functions in
accordance with State Department rules.
THAT, the formation of this District does not imply that the
Member Counties will consolidate the administration of any other
programs of Social Services Departments, Health Departments, or
Department of Institutions.
THAT, Margaret Long, the Director of Garfield County Depart-
ment of Social Services, shall act as the contact person for the
District.

THAT, Kathleen Forinash, the Director of Eagle County
Department of Social Services, be and is hereby appointed as Eagle
County's representative to the policy board of the District.
THAT, this resolution is necessary for the public health,
safety and welfare of the citizens of Eagle County, Colorado.
MOVED, READ AND ADOPTED by the Board of County Commissioners
of the County of Eagle, State of Colorado, at its regular meeting
