1. PARTIES
This Amendment (the “Amendment”) to the Original Contract shown on the Signature and Cover
Page for this Amendment (the “Contract”) is entered into by and between the Contractor, and the
State.
2. TERMINOLOGY
Except as specifically modified by this Amendment, all terms used in this Amendment that are
defined in the Contract shall be construed and interpreted in accordance with the Contract.
3. AMENDMENT EFFECTIVE DATE AND TERM
A. Amendment Effective Date
This Amendment shall not be valid or enforceable until the Amendment Effective Date
shown under the State Controller Signature. The State shall not be bound by any provision of
this Amendment before that Amendment Effective Date, and shall have no obligation to pay
Contractor for any Work performed or expense incurred under this Amendment either before
or after of the Amendment term shown in §3.B of this Amendment.
B. Amendment Term
The Parties’ respective performances under this Amendment and the changes to the Contract
contained herein shall commence on the Amendment Effective Date shown under the State
Controller Signature or May 1, 2020, whichever is later, and shall terminate on the
termination of the Contract or June 30, 2020, whichever is earlier.
4. PURPOSE
The Parties entered into the agreement to support healthy housing policy efforts and community
engagement leadership in Eagle County.
The Parties now desire to decrease funding and change Statement of Work for the following
reason: Priorites of the Eagle County Housing Dept and Community Development have shifted to
focus on affordable housing guidelines and Comprehensive Plan updates.
5. MODIFICATIONS
The Contract and all prior amendments thereto, if any, are modified as follows:
A. The Contract Maximum Amount table is deleted and replaced with the Current Contract
Maximum Amount table shown on the Signature and Cover Page for this Amendment.
B. The Contract Initial Contract Expiration Date on the Contract’s Signature and Cover Page
is hereby deleted and replaced with the Current Contract Expiration Date shown on the
Signature and Cover Page for this Amendment.
C. The Parties now agree to modify Exhibit B, of the agreement. Exhibit B, Statement of Work
is deleted and replaced in its entirety with Exhibit B, Statement of Work attached to this
Amendment for the following reason: Priorities of the Eagle County Housing Department

the following reason: Reduce budget due to shift in focus of priorities.
6. LIMITS OF EFFECT AND ORDER OF PRECEDENCE
This Amendment is incorporated by reference into the Contract, and the Contract and all prior
amendments or other modifications to the Contract, if any, remain in full force and effect except
as specifically modified in this Amendment. Except for the Special Provisions contained in the
Contract, in the event of any conflict, inconsistency, variance, or contradiction between the
provisions of this Amendment and any of the provisions of the Contract or any prior modification
to the Contract, the provisions of this Amendment shall in all respects supersede, govern, and
control. The provisions of this Amendment shall only supersede, govern, and control over the
Special Provisions contained in the Contract to the extent that this Amendment specifically
modifies those Special Provisions.

STATEMENT OF WORK
These provisions are to be read and interpreted in conjunction with the provisions of the contract specified above.
I.Entity Name: Eagle County Public Health and Environment
Term: July 1, 2019–June 30, 2020
II.Project Description:
This project serves to support healthy housing policy efforts and community engagement leadership in Eagle County.
Healthy Housing Eagle County will make policy changes to address long standing injustices in our community related
to historical inequities in housing opportunities and also work to increase homeownership. This will take place
through the development of policies or systems to ensure affordable new development and avoid future displacement.
Authentic community engagement practices will be utilized to support all Healthy Housing initiatives.
III.Definitions:

IV. Work Plan:
Goal #1: To build partnerships to mobilize community power and transform systems to advance health equity and
environmental justice.
Objective #1: No later than the expiration date of this contract, the Contractor shall make changes to policies that address
current and long-standing injustices around housing by authentically engaging the community around healthy and affordable
housing barriers and work together to build solutions.
Primary Activity #1 The Contractor shall research policies that support renters and mobile home owners who rent
land and gain support from the community.
Sub-Activities #1
1.The Contractor shall conduct a literature review of policy changes, including: non-retaliation
ordinances, right of first refusal policies, volunteer rent-caps, in order to identify the most
impactful and relevant policies to pursue in Eagle County.
2.The Contractor shall meet with other municipalities that have similar ordinances and
policies already in place to learn about their successes and challenges.
3.The Contractor shall compile a summary document of the literature review and research.
4.The Contractor shall develop a community engagement plan to determine community
readiness and political will for possible policy changes identified by the research.
Primary Activity #2 The Contractor shall develop an enforcement plan for HB19-1309 Mobile Home Park Act.

Sub-Activities #2
1.The Contractor shall participate in a training to learn about the Mobile Home Park Act.
2.The Contractor shall meet with the County Attorney office and other relevant County
Departments and stakeholders to review the new law and determine additional capacity needs
for meaningful enforcement.
3.The Contractor shall develop a community engagement plan to inform mobile home park
residents of the new law and their associated rights.
Primary Activity #3 The Contractor shall facilitate the development of a community land trust.
Sub-Activities #3
1.The Contractor shall identify key stakeholders for community engagement regarding the
community land trust.
2.The Contractor shall develop a community engagement plan to meet with key stakeholders
in order to gain buy-in for the community land trust.
Primary Activity #4 The Contractor shall develop an action plan to build community power among informal
neighborhood groups.
Sub-Activities #4
1.The Contractor shall hire a subcontractor to provide expertise to build the capacity of
community-based organizations for advocacy and community organizing.
2.The Contractor shall identify individuals and groups that want to build more power and
meaningful voice in decision-making processes.
3.The Contractor shall conduct an assessment of opportunities to build community power
among marginalized groups in Eagle County.
4.The Contractor shall develop an action plan to build community power based on the
assessment that includes identification of resources, tools and trainings needed.
5.The Contractor shall develop a community engagement plan to determine community
interest to develop a Dotsero Metro Board.
6.The Contractor shall provide a minimum of two (2) charlas around the theme of “Mi Salud,
Mi Charco” [My Health, My Puddle (place)] in which residents are interviewed and
participate in designing a solution to their issues using design thinking practices.
7.The Contractor shall provide training for community leaders to learn how they can apply
systems thinking to address health in the built environment (for example, ‘Habits of a
Systems Thinker’ which encompasses a spectrum of strategies that foster problem solving
and thoughtful reflection about systems of interest, in this particular case: place and health).
Primary Activity #5
The Contractor shall develop an action plan based on community-identified needs to implement
key infrastructure improvements in West Edwards to better meet existing and future needs
based on anticipated growth in the area.
Sub-Activities #5
1.The Contractor shall meet with leaders across Eagle County Government Departments and
other relevant stakeholders to clearly identify key challenges and opportunities in West
Edwards with respect to key infrastructure improvements.
2.The Contractor shall attend Edwards Metro Board meetings, as appropriate, to develop a
relationship with Board members and to learn about progress with development proposals.
3.The Contractor shall facilitate discussions between public and private organizations to
address water quality concerns in Eagle River Village, a mobile home park comprised of 381
homes situated in West Edwards.
4.The Contractor shall convene a meeting with Eagle County Government leaders and
stakeholders to develop a viable action plan based on stakeholder meetings, discussions
regarding Eagle River Village, and community engagement activities.
5.The Contractor shall develop a community engagement plan for proposed changes in the
action plan for the West Edwards area.

Standards and
Requirements
1.The Contractor shall monitor documents and website content for updates and comply with
all updates as the content of electronic documents located on CDPHE and non-CDPHE
website and information contained on CDPHE and non-CDPHE website may be updated
periodically during the contract term.
2.The Contractor shall develop quarterly progress reports on the template provided by
CDPHE and follow the Quarterly Reporting Guidance. This document is incorporated, made
part of this contract by reference and is available on the following website which has been
incorporated into this contract: https://drive.google.com/file/d/0B-

3.The Contractor shall align efforts as required by the HDGP evaluation contractor and
CDPHE to include:
a.using specific tools, resources, templates and guidelines;
b.providing baseline data, metrics, measures and progress reporting via a reporting tool
provided by the HDGP evaluation contractor;
c.adhering to established timelines and schedules;
d.participating in evaluation reporting and other data submissions;
e.participating in evaluation-related trainings and technical assistance;
f.providing access to program information and data as needed by the HDGP evaluation
contractor;
g.and other evaluation monitoring and data submissions as determined by the evaluation
contractor and HDGP.
4.The Contractor shall align efforts with the HDGP training and technical assistance
contractor and CDPHE to include: leveraging opportunities; sharing information about
learning opportunities; identifying training topics and populating training calendars; and
participating in the HDGP peer learning opportunities, affinity groups and other training and
technical assistance activities.
5.The Contractor shall budget for a minimum of one (1) in-person HDGP peer learning
opportunity for Contractor identified representatives, date(s) and location to be determined.
6.The Contractor shall participate in conference calls with CDPHE staff as needed, including
a minimum of one (1) per quarter.
7.The Contractor shall participate in HDGP Affinity Group meetings, including a minimum
of one (1) per quarter.
8.The Contractor shall adhere with the National CLAS Standards. This standard is
incorporated, made part of this contract by reference and is available on the following
website: https://www.thinkculturalhealth.hhs.gov/clas/standards.
9.The Contractor shall provide CDPHE, upon request, written procedures related to gift card
purchase and handling. At a minimum, the procedures must include the following:
a.How the gift card inventory is tracked and maintained;
b.Gift Card storage and safeguards against theft;
c.The primary person responsible for securing and distributing gift cards;
d. A gift card distribution log that records each gift card number, dollar amount and the
printed name and signature of each gift card recipient.

V.Monitoring:
CDPHE’s monitoring of this contract for compliance with performance requirements will be conducted throughout
the contract period by the Health Disparities Grant Program Manager and Fiscal Officer. Methods used will include a
review of documentation determined by CDPHE to be reflective of performance to include fiscal and programmatic
documentation as applicable. The Contractor’s performance will be evaluated at set intervals and communicated to the
Contractor. A Final Contractor Performance Evaluation will be conducted at the end of the life of the contract.
VI.Resolution of Non-Compliance:
The Contractor will be notified in writing within ten (10) calendar days of discovery of a compliance issue. Within
thirty (30) calendar days of discovery, the Contractor and the State will collaborate, when appropriate, to determine

the action(s) necessary to rectify the compliance issue and determine when the action(s) must be completed. The
action(s) and time line for completion will be documented in writing and agreed to by both parties. If extenuating
circumstances arise that requires an extension to the time line, the Contractor must email a request to the Health
Disparities Grant Program Manager and receive approval for a new due date. The State will oversee the
completion/implementation of the action(s) to ensure time lines are met and the issue(s) is resolved. If the Contractor
demonstrates inaction or disregard for the agreed upon compliance resolution plan, the State may exercise its rights
under the provisions of this contract.
