AND
MOUNTAIN YOUTH (formerly Eagle River Youth Coalition)
THIS SECOND AMENDMENT (“Second Amendment”) is effective as of ________________,
by and between Mountain Youth, a Colorado nonprofit corporation (hereinafter “Consultant” or
“Contractor” and Eagle County, Colorado, a body corporate and politic (hereinafter “County”).
RECITALS
WHEREAS, County and Consultant entered into an agreement dated the 14th day of August,
2018, for certain Services (the “Original Agreement”); and
WHEREAS, the Original Agreement contemplated that the Consultant would perform certain
Services with compensation in an amount not to exceed $80,000; and
WHEREAS, by a First Amendment dated the 15th day of September, 2019 the parties extended
the term for an additional year and amended the scope and compensation of the Original
Agreement; and
WHEREAS, the County desires to have Consultant perform additional Services for additional
compensation as set forth below; and
WHEREAS, the term of the Original Agreement expires on the 30th day of June, 2020, and the
parties desire to extend the term for an additional year.
SECOND AMENDMENT
NOW THEREFORE, in consideration of the foregoing and the mutual rights and obligations as
set forth below, the parties agree as follows:
1.The Original Agreement shall be amended to include additional Services as described
in Exhibit 1, which is attached hereto and incorporated herein by reference.
2. The compensation for the additional Services set forth in Exhibit 1 shall not exceed
$80,000 or a total maximum compensation under the Original Agreement, First
Amendment, and this Second Amendment of $240,000.
3.The term of the Original Agreement is hereby extended to the 30th day of June, 2021.
4.Capitalized terms in this Second Amendment will have the same meaning as in the
Original Agreement. To the extent that the terms and provisions of the Second
Amendment conflict with, modify or supplement portions of the Original Agreement,
the terms and provisions contained in this Second Amendment shall govern and
control the rights and obligations of the parties.

5. Except as expressly altered, modified and changed in this Second Amendment, all
terms and provisions of the Original Agreement shall remain in full force and effect,
and are hereby ratified and confirmed in all respects as of the date hereof.
6. This Second Amendment shall be binding on the parties hereto, their heirs, executors,
successors, and assigns.

SCOPE OF SERVICES, SCHEDULE, FEES
Purpose
Communities that Care (CTC) is an evidence-based approach for communities to form or partner
with existing coalitions, identify prevention priorities and effective strategies to address
concerns, and mobilize the community to implement strategies. CTC funding aligns with Eagle
County and community priorities of substance use prevention and mental health promotion while
avoiding duplication by working with and through existing infrastructure. Resources from this
funding opportunity will enable Eagle County Department of Public Health and Environment
(ECPHE) and Mountain Youth (Contractor) to work together to implement collective strategies
to deter and prevent youth substance use and access. Communities that Care includes five (5)
phases of implementation: (a) Phase 1: Get Started; (b) Phase 2: Get Involved; (c) Phase 3:
Develop Community Profile; (d) Phase 4: Create a Plan; and (e) Phase 5: Implement and
Evaluate.
Scope of Services
1. The Contractor shall provide a 0.8FTE CTC Local Coordinator.
2. The Contractor shall reimburse and/or incentivize CTC coalition members for direct
contributions as requested, as well as vendors for CTC-related events.
3. The Contractor shall work with the ECPHE CTC co-lead to recruit key community leaders, as
well as maintain community boards and workgroups.
4. The Contractor shall use effective group facilitation skills and guide the coalition through the
CTC planning and implementation process.
5. The Contractor shall lead completion of the Milestones and Benchmarks tool in consultation
with ECPHE CTC co-lead with input from the community board as a measure of progress and
fidelity to the CTC process.
6. The Contractor shall be the primary lead of the CTC workshops required throughout the
process, in appropriate order and timing for effective community progress, with support from the
Colorado Department of Public Health and Environment (CDPHE) appointed CTC coach and
ECPHE CTC co-lead.
7. The Contractor shall handle community board meeting scheduling, preparation, and follow-up.
8. The Contractor shall provide oversight and technical support for workgroup and
implementation team leadership to ensure progress according to CTC process.
9. The Contractor shall hold regular meetings of the community board and key leader board, at a
frequency dictated by the CTC process under CDPHE guidance.
10. The Contractor shall build capacity of community coalition members to address mental
health concerns, and alcohol, prescription drug, and other substance use/abuse among youth by
offering a minimum of two (2) trainings designed for coalition members: youth mental health
and development, substances and impacts of their use on young people, and/or positive youth
development principles and practices.
11. The Contractor shall review new data about substance abuse and mental health risk and
protective factors in the community using a list provided by CDPHE of recommended indicators.
This shall include school, district, or regional-level data from the Healthy Kids Colorado Survey.
12. The Contractor shall periodically review and update information included in the existing
local community health assessment regarding youth substance abuse and mental health with

members of the coalition and ECPHE CTC co-lead, to ensure coordination with other existing
assessment efforts.
13. The Contractor shall complete additional assessments/data analysis if deemed necessary after
other community assessments have been reviewed and considered, to develop a clear picture and
analyze gaps in youth substance abuse prevention and mental health within the community,
specifically related to shared risk and protective factors that prevent substance use and improve
mental health.
14. The Contractor shall continue to work with ECPHE CTC co-lead to update map of existing
resources already in place within the community and current strategies for implementation
adopted by the CTC coalition to address the gaps identified in the data, including programs
funded by Colorado Department of Education, Maternal and Child Health, Tony Grampsas
Youth Services, Office of Behavioral Health, Colorado Department of Transportation, or
Department of Criminal Justice.
15. The Contractor shall periodically analyze and summarize these assessment results in an
updated community health assessment report using a CDPHE-approved template for the
substance abuse-related risk, protective, and outcome data, and arrange for public dissemination
through the CTC community board and key leader board.
16. The Contractor shall support community-wide distribution of the Healthy Kids Colorado
Survey results in Spanish and English.
17. The Contractor shall co-lead dissemination of CTC information to key leaders, schools and
other youth service providers and the public, including making presentations and preparing
community board members for community outreach.
18. The Contractor shall communicate often and effectively with ECPHE CTC co-lead to
collaborate and coordinate on CTC planning, implementation, and maintenance activities.
19. The Contractor shall prepare documentation and supporting materials for reports as needed.
20. The Contractor shall assist in writing grant proposals, reports and other materials needed to
obtain funding.
21. The Contractor shall provide ECPHE contract monitor with an updated staff roster of CTC
Local Coordinator within two weeks of any staffing change.
22. The Contractor shall discuss edits and updates to the community prevention action plan with
the ECPHE CTC co-lead.
23. The Contractor shall support the positioning of the CTC process as a driver of prevention and
education aspects of the community-wide behavioral health systems work (Eagle Valley
Behavioral Health Initiative).
Monthly Reimbursements
A monthly invoice will be emailed by Mountain Youth to phinvoices@eaglecounty.us by the
15th of each month. Invoices must include actual personnel expenses and itemized expenses
identified below.
• Personnel costs include (total $59,000):
Facilitator, including Fringe Benefits ($52,000)
Lead Youth Advisor ($3,000)
CTC Members claiming hourly reimbursement ($1,000)
Facilitator’s Supervision ($3,000)
• Meeting costs for child care, food ($1,000)
• Meeting incentives including gift cards for members too young to claim hourly reimbursement,

mileage reimbursements or transportation vouchers for CTC members as requested and
allowable ($500)
• Local and state travel for personnel to attend meetings and required trainings ($1,500: $500
routine meeting mileage; $1,000 conference travel including Risk and Protective Factors
conference)
• Office supplies, equipment and materials for use in meetings and events such as easel paper,
writing instruments, sticky notes, folders, CTC personnel phone and internet expenses, etc.
($1,500)
• Community-level strategy implementation targeting CTC-prioritized risk and protective factors
($10,500):
● compensation for youth involved with: implementing local marketing of mental health
and substance use campaigns; presenting to stakeholders to educate on youth substance
use and/or mental health; participating in projects to gather baseline data or research to
inform substance use prevention efforts such as the Project Towards No DUI or
substance ordinance work; participation in Valley’s Voice youth coalition and
development of substance use prevention projects and initiatives.
● content experts to guide a specific aspect of an activity such as: facilitators to deliver
mini-trainings and consultations for organizational and business leaders to understand
Positive Youth Development and how to integrate youth into their work;
● meetings to build relationships with stakeholders such as: organizational and business

• Indirect Rate of 10% to cover portion of payroll, finances, organizational memberships, rent,
etc.($6,000)
The compensation for the Services set forth in Exhibit 1 shall not exceed $80,000.
