Letter of Agreement RE: Eagle County Public Health accepts transference and implementation of the Pitkin
County WIC Innovation Project
This letter serves as the official notice that Eagle County Public Health and Environment will accept the transfer of
the Pitkin County WIC Innovation Grant effective immediately.
Community Health Services will transfer $11,230 to Eagle County WIC program for the remaining of year one and
will transfer the remaining revenue $33,765 upon deliverance by the Colorado Health Foundation for the second
year of implementation by 11/30/18.
Purpose of Project: Improve enrolment and retention efforts to increase the number of enrolled to 80% of those
eligible. This will improve the overall health of our community by increasing the number of children and adults
who have access to healthy food.
Intended measurable results to be reported on:
• Increase the number of children and adults who have convenient access to affordable fruits and
vegetables.
Anticipated Year 1 Quantity: 69
Anticipated Year 2 Quantity: 82
Expected Intermediate Milestones:
1. Engage Pitkin County child care centers and other partners including Community Health Services,
Mountain Family Health Centers, food banks and pantries, SNAP, Valley Settlements (local Roaring Fork
non-profit that works with the Hispanic community and focuses on economic stability and community
engagement for low-income families) to identify education and referral process needs and solutions.
2. In consultation with partners, create paid and earned social media, media, and marketing campaign.
3. Increase the number of eligible children served by six (year one) and by ten (year two), and women by
three (year one) and four (year two).
