HomeMy WebLinkAboutC18-005 Larimer County Department of Human ServicesMemorandum of Understanding Between Larimer County Department of Human Services and Eagle County Colorado for CROP (SAMHSA Grant) This Memorandum of Understanding (MOU) is entered into on 01/09/2018 , by and between Larimer County Department of Human Services (LCDHS), and Eagle County, Colorado (Eagle County) a body corporate and politic by and through its Board of County Commissioners. LCDHS and Eagle County shall jointly be referred to as the "Parties." The following is to confirm the intent of the LCDHS to work collaboratively with Eagle County to assist in providing the services outlined in the Colorado Children's Congregate Care Resiliency Opportunity Project (CROP) Substance Abuse and Mental Health Services Administration (SAMHSA) grant, as described herein. Length of Amement: This MOU will be effective July 1, 2017 and shall continue for 15 months until September 29, 2018, unless terminated as provided for in paragraph two of `other conditions'. This MOU automatically renews each year on October 1, and continues until September 29, 2021 unless modified by the Parties in writing or earlier terminated. General Purpose: LCDHS, in partnership through an operational agreement with Colorado State University (CSU) and its Department of Human Development and Family Studies, will act as a team with Eagle County to build infrastructure (staff training, development of an assessment model, and train and support Endorsed Providers as defined below), develop an assessment model, conduct training for clinicians to become Endorsed Providers (as discussed below), and conduct training for both clinical and casework staff to build trauma informed practices. Child Trauma and Resilience Assessment Center (CTRAC), housed within the Department of Human Development and Family Studies, serves as a local, state, and regional child trauma assessment center. CTRAC will conduct trauma assessments (Trauma Assessments) throughout the Parties' counties. The current assessment model is for Children 0- 18. During the course of the CROP project, the Parties will work cooperatively to develop adult screening and assessment protocols. Once the Parties establish protocols and procedures, they will have the option to refer for both children and adults for Trauma Assessments pursuant to this MOU. All parties agree to adhere to the terms identified in Exhibit A. The SAMHSA grant funds are also available for Eagle County clinicians to proceed through a training process provided by CSU and CTRAC. Once these Eagle County clinicians have completed the training and observation process and they meet various requirements, these clinicians (the Endorsed Providers) are certified by CTRAC to conduct Trauma Assessments. The total funds available to train potential Endorsed Providers may vary each year dependent on the CROP budget and goals. Individual county access to these dollars is defined in Exhibit B. . The Treatment Outcome Package (TOP) is a well-being measurement tool used to measure change/progress for children as they respond to the various recommendations provided in the Trauma Assessments. The Parties have agreed to the use the TOP as a measurement tool for children in the eligible population for the SAMHSA grant. Other Conditions: The Parties agree to comply with federal, state and local laws and regulations including all requirements for safeguarding client information. All shared information that is not subject to the Colorado Open Records Act will be treated as confidential information. All services shall be provided to eligible persons without regard to race, gender, religion or ethnic status. Any modification to this MOU must be in writing and signed by all parties. CTRAC will perform Trauma Assessments in Eagle County and provide grant coordination. Services provided under the CROP (SAMHSA) grant by CTRAC will be covered by the grant. Eagle County agrees to do to the following: 1. Ensure all children/youth meet the CROP target population prior to enrollment in grant activities. The population is defined as follows: Mandatory Elements: • Opened Child Welfare case • Active Participating as a Child • Trauma Screen In with Score b or above • TOP or CANS as a baseline for well-being Must meet one of the following: 1. In Congregate Care (currently or in past b months) 2. In Hospital 3. In Juvenile Justice System a. Charged (Truancy, Municipal, County or District Court) b. Pre -Adjudicated c. Adjudicated d. Diversion e. Probation f. Engaged in Competency Restoration 4. Ages 7 — 17 Youth in Conflict 2. Provide client records (health history, prior evaluations, social history, school records. etc.), complete referral documents, and provide all necessary releases of information and consent for treatment for clients referred for a Trauma Assessment with CTRAC. 3. Ensure children/youth are made available for the scheduled Trauma Assessment as well as coordinate appropriate space in Eagle County for assessments to take place. Eagle County is responsible for gathering all required pre -assessment referral information and having a `back up' child selected for an assessment in the event the scheduled child/youth cannot be assessed. In the event CTRAC is scheduled to conduct an assessment and no child/youth is able to be present this will be considered a No Show and/or Cancelation. It is the responsibility of Eagle County to notify CTRAC no later than 3 business days prior to the scheduled assessment of the Cancelation if a backup child/youth cannot be identified. Failure to do so will result in a forfeit of the assessment by Eagle County and it may be asked to reimburse the grant the cost for the assessment teams to travel expenses. 4. Provide trauma services and follow recommendations for completed Trauma Assessments. M Exhibit B Funds are available under the SAMHSA grant for Endorsed Providers to conduct Trauma Assessments in addition to Trauma Assessments conducted by CTRAC. The total funds available may vary each year dependent on the CROP budget and goals. Eagle County will be provided a total number of assessments available during the first quarter of each grant year (Oct 1 -Dec 31). This number will be based upon a percentage of its target population to be served. The reimbursement rate for the Trauma Assessment provided under the CROP grant will be up to $1900 per assessment, but will not exceed each county's contracted rate with the Endorsed Provider. Eagle County agrees to the following: 1. Eagle County and CTRAC will pre-screen the ChildrenlYouth assessed by an Endorsed Provider in accordance with the requirements set forth in paragraph 1 in Exhibit A for grant population compliance. 2. If Eagle County does not use CTRAC to conduct the Trauma Assessments, it shall only utilize Endorsed Providers for the SAMHSA grant Trauma Assessments. 3. Eagle County will adhere to data sharing requirements between Eagle County, CTRAC and LCDHS. Eagle County, CTRAC and LCDHS will develop these data sharing requirements, which may include release of client names, Date of Birth, Trails ID, justification of eligible criteria regarding children assessed by Endorsed Providers. Eagle County will provide Trauma Assessment result data for children to CTRAC and LCHDS through the Lime Survey data collection process, which include, but is not limited to, data regarding: specific recommendations; client follow through with recommendations; and quality assurance surveys completed by Eagle County to monitor the fidelity to the Trauma Assessment. Eagle County will provide TOP data to CTRAC and LCDHS for eligible children. 4. Eagle County will submit an invoice to LCDHS no later than 60 days from completion of the Trauma Assessment for reimbursement. LCDHS may request invoices early at the end of the grant period as grant funds may not be available for carryover. The invoice must include the following information: a. Child Name, DOB, Trails Client ID b. Endorsed Provider Name c. Date of Trauma Assessment d. Reimbursement rate of $1900 or the lesser amount if the contracted rate is less than $1900. 5. Eagle County and Endorsed Provider will collaborate to ensure that any recommendation contained in a Trauma Assessment is a service that is available for Eagle County residents. b. Eagle County agrees to provide trauma services and follow the recommendations made by Endorsed Providers in completed Trauma Assessments to the extent such services are available in or around Eagle County. 4
