I acknowledge that 1 have requested U S West Communications, Inc. CRISWC� to supply Sham Payphone
Service according to the tains and conditions of the applicable Tariff or Catalog.
1.TERIVI. This Agreement shall be effective upon the dam of signature below and shall remain in effect
for a period of one year.
2. REVENUE DISTRIBUTION. When revenue is collecred from the Payphone. USWC's local and
inu-A ATA revenue ("Revenue'? will be allocated as follows: a dollar amount equal to the monthly service
charge. will be retained by USWC as revenue and wiII not be applied towards payment of current month's
service charge. From the balance of Revenue collecred during that collection period, an additional dollar
amount equal to the service charge will be applied as a credit toward payment of current month's service
charge. Should a balance of Revenue still remain. a 59b commission will be credited toward next month's
service charge. In the event there is a balance due to USWC after the Revenue distribution, I agree
payment on or before the date specified in the monthly statement.
3. TERMINATION CHARGES. If I temnare Shared Payphone Service prior to expiration or term
or fail to make payment, USWC may terminate Service, remove Payphone and bill termination charges as
shown below upon providing ten (10) days written notice.

4. ARBITRATION. Any controversy or claim relating to Shared Payphone Service shall be settled
by binding arbitration under the rules of the American Arbitration Association. Judgment upon the award
