this reference made a part hereof), between the State of Colorado Department of Public Health and Environment,
SP ial Supplem-ntal Nut -Mcn Pi-ogram for Wome.y ;nfiaa�s, and Chiidraji MC Program) and_EAg Cly
Board of County Commissioners covering the period October 1 -19
Qfi_through September 30 . 19-2$_ the undersigned agree that the maximum amount payable by the
State for WIC services is being decreased by $ 1.132 to a new total of $_55.576 and is to be spent
in the following manner.
October 1, 19-2L through December 30, 19-9&: $ 14.177
January 1,19M through March 31, 19M: $ 13.800
April 1, 19-M through June 30, 19M: S-13.800
July 1, 19M through September 30, 19M: $13.799
Paragraphs 27 &Z$ are hereby modified accordingly. The terms and conditions of the original contract shall
remain the same.
This amendment to the contract is intended to be effective as of_lQ_4j_Ql, _2L but in no event shall it be deemed
valid until it shall bm been am=d by the State Controlla or his &Zborized dainee.
