In accordance with Paragraph 13 of contract routing number 96-05124 of contract number EP19605124, and
contract routing number 97-05206 of Change Order Letter No. 1(copies attached and by this reference made
a part hereof) between the State of Colorado Department of Public Health and Environment (Disease Control
and Environmental Epidemiology Division) and Eagle County Board of Commissioners covering the period
of January 1, 1997 through December 31, 1997 the undersigned agree that the maximum amount payable by
the State for eligible services is Three Thousand Five Hundred Sixty -Seven Dollars ($3,567). Funding is
contingent upon continued availability of federal funds for payment pursuant to the terms of this agreement..
The terms and conditions of the original contract shall remain the same.
This amendment to the contract is intended to be effective 9s of 01/01/97, but in nn event shall it he dePmm
valid until it shall have been annrnved by the State rantmller nr such assistant as he may designate
