CONTRACT RENEWAL LETTER and Environment
(Approved Waiver)

According to Paragraph #10 of contract FAACPD 960233, Routing Number 960233 (copy attached and by this
reference made a part hereof) between the State of Colorado, Department of Public Health and Environment and
Eagle Board of County Commissioners, the State will cause to be paid to the Contractor the sum of fifteen
thousand, three hundred and sixty-two ($15,362) to be reimbursed monthly based on a sum equal to 1/12 of the
contract amount upon receipt of signed affidavits or expenditures beginning July 1, 1997 and ending June 30,
1998.
Payment pursuant to this contract will be made as earned, in whole or in part, from available State funds
encumbered in an amount not to exceed two hundred and twenty-two thousand, three hundred and thirty-eight
dollars ($222,338) for the purchase of County Environmental Health Services. This amount was determined using
the 1998 Long Bill appropriation under Local Health Services for Public Health Sanitarians in areas not served
by local and regional health departments. The terms and conditions of the original contract shall remain the same.
This letter of amendment is intended to be effective July 1, 1997, but in no event shall it be deemed valid until
it shall have been approved by the State Controller or such assistant as he may designate.
Please sign all four (4) copies of this letter and return to Julie Akin at the address below by June 23, 1997.
