PURCHASE OF SERVICE CONTRACT
EAGLE COUNTY WRAP PROGRAM
1. THIS CONTRACT, made this 1st day of October, 1997_, by and between the Eagle County
Department of Health and Human Services, hereinafter called "ECHHS" and Colorado
West Regional Mental Health (provider): 6916 Highway 82. Glenwood Springs. CO 81601
(Address); 84-0625 890 (tax ID or Social Security number), hereinafter called "Contractor".
2. This Contract will be effective from October 1 , 1997 until June 30, 1998_
3. ECDSS agrees to purchase and Contractor agrees to provide counseling - individual,
group, and family: assessment: testing, psychiatric evaluations, and mentoring (Services) for
Youth/families referred for specific services by the Eagle WRAP committee.
4. ECDSS agrees to purchase and Contractor agrees to furnish hourly Units of Service at an
hourly rate of $20 -50 -/hour (with a minimum billing unit of .25 hour) or a service fee of
Un to $100.00 for a maximum amount of this contract of $20,000. (Service rates will vary
by type of service provided but will not exceed $50/hr).
5. The parties agree that the Contractor's relationship to the county is that of an independent
contractor.
6. The parties agree that payment pursuant to this Contract is subject to and contingent upon the
continuing availability of fund for the purpose thereof.
7. ECDSS agrees:
a. to determine child eligibility and, as appropriate, to provide information regarding
rights to fair hearings
b. to provide contractor with prior authorization, on a child or family basis, for services to
be purchased.
c. to provide Contractor with referral information including name and address of family,
social, medical and educational information as appropriate to the referral.
d. to assess and collect fees as appropriate.
e. to monitor the provision of contracted service.
f. to pay Contractor after receipt of billing statements for services rendered satisfactorily
and in accordance with this Contract.
8. Contractor agrees:
a. not to assign any provision of this Contract to a subcontractor.
b. Not to charge clients any fees in relation to services provided under this Contract.

c. to comply with the requirements of the Civil Rights Act of 1964 and Section 504,
Rehabilitation Act of 1973 concerning discrimination on the basis of race, color, sex, age,
religion, political beliefs, national origin, or handicap.
d. to provide the service described herein at cost not greater than that charged to other
persons in the same community.
e. to submit a billing statement in a timely manner, no later than 45 days after services.
Failure to do so may result in nonpayment.
f. to safeguard information and confidentiality of the child and the child's family in
accordance with rules of the Colorado Department of Human Services and the Eagle County
Department of Health and Human Services.
g. to provide youth/family progress in program to WRAP team as requested.
h. to provide access for any duly authorized representative of the ECDSS or the Colorado
Department of Criminal Justice until the expiration of five (5) years after the final payment under
this Contract, to examine any financial, program, and other records of the Contractor involving
transactions related to this Contract.
j. within the limits of, and without expanding or multiplying the limits of governmental
immunity, indemnify Eagle County, Colorado Department of Human Services and the State of
Colorado against any liability and loss against all claims and actions based upon or arising out of
damage or injury, including death, to persons or property caused or sustained by Contractor's
performance of this Contract.
9. Termination: Either part may terminate this Contract by thirty (30) days prior notification in
writing.
