SUPPLEMENT NO. M
TO THE U S WEST COMMUNICATIONS, INC. PURCHASE ORDER
BETWEEN EAGLE COUNTY GOVERNMENT AND
U S WEST COMMUNICATIONS, INC.
U S WEST Communications, Inc. ("USWC'), and Eagle County Government hereby enter
into this Supplement No.1_ ("Supplement") to the U S WEST Communications, Inc. Purchase Order
No.FRSR 176339 ("Purchase Order"), dated 9/22/97.
Section 1. Purpose Of This Supplement. The purpose of this Supplement is to add
additional locations.
Section 2. Additional Locations. The additional locations are being added as follows:
upgrade One 56K circuit with 4 PVC at 500 Braodway, Eagle, CO to
One T1 circuit with 4PVCs at Broadway, Eagle, CO
Change one 56K circuit with 1 PVC at 2250 Hwy 131, Wolcott, CO to
One 56 K circuit with 1 PVC at 0219 Eldon Wilson Drive, Gypsum, CO 81637
Section 3. Terms And Conditions. Except as modified herein, the terms and conditions
of the Purchase Order govern this Supplement and the Frame Relay I Service supplied under this
Supplement.
