to the agreement made effective December 21, 2016 by and between Northwest
Colorado Council of Governments, 249 Warren Ave., P. O. Box 2308, Silverthorne, GC
80498-2308 and Eagle County, Colorado through its Public Health and Environment
Department, PO Box 660, Eagle, CO 81631 for the Older American Act services of
Congregate and Home Delivered Meals.
AGREEMENT PERIOD, The period of performance mirrors the contract, paragraph 6.
NWCCOG will increase the contract amount for Congregate Meals by $14,283.99 to a
total of $73,512.99 and will increase the contract amount for Home Delivered Meals by
$10,529.46 to a total of $24,519.46 This funding is in addition to the original contract
amount and must be fully expended by June 30, 2017.
