EAGLE COUNTY AND GRANT RECIPIENT

Whereas,Eagle County works to promote the health,safety,and welfare of Eagle County citizens of all
ages;and
Whereas,among the services Eagle County provides in order to promote such health,safety,and welfare
are grants-in-aid to local organizations whose work assists the County in accomplishment of the goal to
create and sustain a safe,healthy,and socially diverse community;and
Whereas,Catholic Charities,Western Slope Regional Office (hereinafter referred to as “Grant Recipient”)
has agreed to provide services that create and sustain a healthy and socially diverse community in Eagle
County;and
Whereas,Grant Recipient acknowledges that pursuant to the terms of C.R.S.§24-76.5-101,et seq.,Eagle
County is prohibited from providing certain public benefits to illegal aliens over the age of eighteen.
Now,therefore,in consideration of the foregoing premises and the following promises,Eagle County and
Grant Recipient enter into this Agreement for payment of the grant award in the amount of Fifteen
Thousand Dollars ($15,000)to Grant Recipient.
A.Grant Recipient agrees that funds awarded by Eagle County may be used and expended solely for
Catholic Charities’Emergency Assistance Program to provide financial assistance as well as case
management that addresses the cause of a crisis in order to prevent homelessness among low-income,at-
risk households in Eagle County as specified in the 2013 Community Service Grant Application and
Grant Application Budget Section.All funds will be expended in the specified grant period.
B.In return for the award of the $15,000.00 grant,Grant Recipient agrees to submit a report
discussing how the funds were expended,deliverables as outlined in the Grant Application,and how they
tie back to creating and sustaining a healthy and socially diverse community.The report will be due no
later than December 31,2013 or submitted along with the submission of your application for 2014,
whichever is first.In addition,if work under the Grant is performed between submission of an application
for 2014 and December 31,2013 a supplemental report can be requested.Grant Recipient agrees to use a
modified version of the Colorado Common Grant (CCG)Report format which must be downloaded and
printed from the following website:http://www.eaglecounty.us/HHS!Cornmunity Service Grants!.
C.Payment of the Grant Amount will be made in one installment.
D.Grant funds under this Grant Agreement are to be used and expended within the Grant Period.
Any funds not used or expended during the Grant Period must be returned to Eagle County.
E.If,after payment of the Grant Award,Eagle County reasonably determines that payment was
improper because the purposes for which the payment was made were misrepresented,or the terms of this
Agreement were breached,then upon written notice of such determination and request for reimbursement
from Eagle County,Grant Recipient shall forthwith return such payment to Eagle County.
F.In the event of litigation in connection with this Agreement,the prevailing party shall be entitled
to recover all reasonable costs incurred,including attorney fees,costs,staff time and other claim related
expenses.
G.Grant Recipient shall comply at all times and in all respects with all applicable federal,state and
local laws,resolutions and codes;and specifically,with the requirements of the Civil Rights Act of 1964
and Section 504,Rehabilitation Act of 1973,concerning discrimination on the basis of race,color,sex,
age,religion,political beliefs,national origin or handicap.
H.Grant Recipient shall not assign any of its rights or duties under this Grant Agreement to a third
party without the prior written consent of County.County shall terminate this Grant Agreement in the
event of any assignment without its prior written consent,in which case,any unused Grant Funds must be
returned to County.
I.Grant Recipient agrees that no agent,employee or volunteer of Grant Recipient shall be deemed
an agent,employee or volunteer of County.
J.Grant Recipient shall maintain,for a minimum of three (3)years,adequate financial and
programmatic records for reporting to County on performance of its responsibilities hereunder.Grant
Recipient shall be subject to financial audit by federal,state or county auditors or their designees.Grant
Recipient authorizes County to perform audits or to make inspections during normal business hours,upon
48 hour notice to Grant Recipient for the purpose of evaluating performance under this Grant Agreement.
Grant Recipient shall cooperate fully with authorized Health and Human Services representatives in the
observation and evaluation of the program and records.Grant Recipient shall have the right to dispute
any claims of misuse of funds and seek an amicable resolution with County.
K.This Agreement shall be governed by the laws of the State of Colorado.Jurisdiction and venue
for any suit,right or cause of action arising under,or in connection with this Agreement shall be exclusive
in Eagle County,Colorado.
L.This Grant Agreement supersedes all previous communications,negotiations,and/or agreements
between the respective parties hereto,either verbal,or written,and the same not expressly contained
herein are hereby withdrawn and annulled.
M.This Grant Agreement does not,and shall not be deemed or construed to confer upon or grant to
any third party or parties any right to claim damages or to bring any suit,action or other proceeding
against either Grant Recipient or County because of any breach hereof or because of any of the terms,
covenants,agreements and conditions contained herein.
N.Invalidity or unenforceability of any provision of this Grant Agreement shall not affect the other
provisions hereof and this Grant Agreement shall be construed as if such invalid or unenforceable
provision was omitted.
0.Grant recipient agrees to the following:
(a)Mandatory Reporting.Grant Recipient shall notify Health and Human Services
(“HHS”)immediately of all reports of suspected child abuse or neglect involving Grant
Recipient,including,but not limited to,employees,volunteers and clients.HHS partners
are considered to be mandatory reporters for suspected child abuse and neglect and are to
make those reports directly to HHS Children and family Services -(970)328-8840.
P.Grant Recipient shall carry workers compensation and such other insurance as is necessary in
carrying out its performance under the Grant Agreement.
Q.If Grant Recipient (hereinafter “Contractor”for purposes of this Paragraph Q)has any
employees or subcontractors,Contractor shall comply with C.R.S.§8-17.5-101,et seq.,
regarding Illegal Aliens —Public Contracts for Services,and this Contract.By execution of this
Contract,Contractor certifies that it does not knowingly employ or contract with an illegal alien
who will perform under this Contract and that Contractor will participate in the E-verify Program
or other Department of Labor and Employment program (“Department Program”)in order to
confirm the eligibility of all employees who are newly hired for employment to perform work
under this Contract.
(a)Contractor shall not:
(i)Knowingly employ or contract with an illegal alien to perform work under
this contract for services;or
(ii)Enter into a contract with a subcontractor that fails to certify to the
Contractor that the subcontractor shall not knowingly employ or contract with an illegal
alien to perform work under the public contract for services.
(b)Contractor has confirmed the employment eligibility of all employees who
are newly hired for employment to perform work under this Contract
through participation in the E-verify Program or Department Program,as
administered by the United States Department of Homeland Security.

(c)The Contractor shall not use either the E-verify program or other Department
Program procedures to undertake pre-employment screening of job applicants while the
public contract for services is being performed.
(d)If the Contractor obtains actual knowledge that a subcontractor performing work
under the public contract for services knowingly employs or contracts with an illegal
alien,the Contractor shall be required to:
(i)Notify the subcontractor and the County within three (3)days that the
contractor has actual knowledge that the subcontractor is employing or contracting with
an illegal alien;and
(ii)Terminate the subcontract with the subcontractor if within three (3)days
of receiving the notice required pursuant to subparagraph (i)of paragraph (d)the
subcontractor does not stop employing or contracting with the illegal alien;except that
the Contractor shall not terminate the contract with the subcontractor if during such three
days the subcontractor provides information to establish that the subcontractor has not
knowingly employed or contracted with an illegal alien.
(e)The Contractor shall comply with any reasonable request by the Department of
Labor and Employment made in the course of an investigation that the department is
undertaking pursuant to its authority established in C.R.S.§8-17.5.102(5).
(f)If a Contractor violates these prohibitions,the County may terminate the contract
for a breach of the contract.If the contract is so terminated specifically for a breach of
this provision of this Contract,the Contractor shall be liable for actual and consequential
damages to the County as required by law.
(g)The County will notify the office of the Colorado Secretary of State if Contractor
violates this provision of this Contract and the County terminates the Contract for such
breach.
R.Grant Recipient shall indemnify County for,and hold and defend the County and its
officials,boards,officers,principals and employees harmless from all costs,claims and
expenses,including reasonable attorney’s fees,arising from claims of any nature whatsoever
made by any person in connection with the negligent acts or omissions of,or presentations by,
the Grant Recipient in violation of the terms and conditions of this Grant Agreement.
S.The signatories aver that to their knowledge,no employee of the County has any personal
or beneficial interest whatsoever in the service or property described in this Grant.The Grantee
has no interest and shall not acquire any interest,direct or indirect,that would conflict in any
manner or degree with the performance of Grantee’s services and Grantee shall not employ any
person having such known interests.
T.Grant Recipient certifies that it has read the Agreement,understands each and every term and the
requirements set forth herein,and agrees to comply with the same.
Funds will not be disbursed without a signed Grant Recipient Agreement.Please complete this

This grant will go toward the Emergency Assistance and Community Integration Services programs,
which together prevent homelessness and increase self-sufficiency through direct fmancial
assistance,case management and community advocacy.These intertwined programs work to create
a healthy and socially diverse community and align with the Self-Sufficiency and Economic
Supports focus Area because they help bring economic stability to clients’lives and move them
toward selfsufficiency.As part of the proposal,we are asking for $7,500 of the $27,500 request to
go toward the publishing of an area resource guide titled Living in the Eagle River Valley.Through
conversations and surveys,area organizations and individuals have identified lack of awareness as a
major barrier to accessing services and resources and expressed the desire and need for a resource
anide to hem eliminate that balTier.

NARRATIVE
1.ORGANIZATION BACKGROUND
Catholic Charities opened its firsi Western Slope office in the basement of the Vail Interfaith
Chapel in 1995,with programs tailored to meet the needs of the local community.The main
office was moved to Glenwood Springs in 2000,and satellite offices are maintained in Avon and
Eagle.The primary local needs identified by Catholic Charities included homeless prevention
services and services for the immigrant community.To this end,Catholic Charities has provided
its Emergency Assistance,Community Integration Services and Transitional Housing programs
for more than a decade.As an agency,Catholic Charities provides supportive,safety-net services
to people of all faiths,backgrounds and cultures.Catholic Charities started out as a Depression-
era emergency assistance program,and although it has evolved over the years to meet society’s
changing needs,its purpose of working with the vulnerable and the poor to alleviate poverty and
promote self-reliance has remained the same.In addition to its Glenwood Springs office.
Catholic Charities has offices in Denver.Fort Collins and Grecley.Catholic Charities and
Community Services of the Archdiocese of Denver,Inc.,is an independent,nonprofit
corporation.
2.GOALS
The overarching goal of Catholic Charities’Western Slope office is to serve the most vulnerable
residents of Eagle County,thereby preventing homelessness,keeping individuals and Ihrnilies
safe,and empowering individuals/families to attain self-sufficiency.
We have three goals for our Emergency Assistance program in the upcoming year:They are:(1)
To standardize requirements for each of the services provided;(2)To increase resources for
habitual clients,thereby assisting them to reach the agency goal of self-sufficiency;and (3)To
engage in community outreach to increase visibility and increase funding.
We also have three goals for our Community Integration Services program:(1)To increase
community awareness;(2)To increase the immigrant community’s access to community
information and referral services;and (3)To provide clients with case management to help
empower them to live sustainable lives with dignity.
3.CIJRRENT PROGRAMS
Catholic Charities Western Slope office runs three main programs:Emergency Assistance,
Community Integration Services,anti Transitional Housing.The Emergency Assistance and
Community Integration Services programs are discussed below,per application instructions.The
Transitional 1-lousing program helps homeless individuals/families or those facing imminent
homelessness obtain and/or maintain housing and become stable through financial supportive
services,case management and referrals.In the last fiscal year,Catholic Charities assisted 45
families through this program.In the current fiscal year,Catholic Charities expects to help 1$
families through the program.(The reduction is due to the end of a three-year federally funded
grant.)

4.PROCRAM OR PROJECT REQUESTS ONLY
a)Through its Emergency Assistance (EA)and Community Integration Services (CIS)
programs.Catholic Charities,Western Slope Region.addresses the need in Eagle County
for services that increase an individual’s ability to become self-sufficient.The goal of the
EA and CIS programs is to create a strong community,both economically and socially.
This is accomplished by providing residents with economic support in times of crisis and
by ensuring that all residents are connected to,and know how to access,vital community
resources in the health,education and social service fields.
The EA program is designed for people who experience an unexpected financial crisis
that may leave them in danger of losing their housing or not being able to pay everyday
bills.Clients may be unemployed,underemployed or fully employed but unable to keep
up with the high cost of living in the area.The program’s objectives are:(1)prevent
eviction or utility shutoff and provide other emergency assistance to households;and (2)
provide case management,assessment,consultation and/or referrals to 100%of clients
who receive assistance.Clients may be referred to other Catholic Charities services or to
other agencies that can help them overcome their financial emergency and move toward
self-reliance.
The CIS program sees clients on an appointment basis in Avon and Eagle.The program’s
objectives are to help 160 immigrant households:(1)attain workplace equity;(2)increase
their access to community resources;(3)adapt more easily to American culture;and (4)
stay housed.These objectives arc met by providing immigrants with mediation in
disputes with landlords,employers or creditors;direct financial assistance if appropriate;
referrals and case management in accessing community resources and services;and
cultural integration services which help immigrants adjust to their new environment,
stabilize,stay housed and move toward self-reliance.The EA and CIS programs arc
ongoing.
b)Catholic Charities has one staff member in eastern Eagle County who administers both
the EA and CIS programs,through which there is much crossover.She effectively
utilizes each program’s services to achieve maximum impact.Western Eagle County is
served through the Glenwood Springs office.Catholic Charities has found that providing
assessment,consultation and case management along with direct financial assistance is an
effective way of keeping low-income and vulnerable individuals and households
stabilized,housed and moving toward self-reliance.
5.EVALUATION
a)Catholic Charities’overall approach to evaluation is to gather as much information as
possible about each of its programs and continuously make improvements where needed.
Information concerning the impact and progress of each program is reported on a
quarterly basis.Evaluation of a program’s impact and progress is done through Catholic
Charities’internal Performance Quality Improvement (PQI)process,and information
from this process is compiled and reported on a quarterly basis.This information is used
to make needed services adjustments and to develop annual program plans.Reports to
external government and private funders also function as an evaluation component for the

agency.Catholic Charities uses several tools in the PQI process,including client intake
forms,client satisfaction surveys and follow-up phone calls.
b)Success of the EA program is measured by tracking how many households receive
assistance,both financially and through case management and,as a result,prevent
homelessness.The CIS program has components that are more subjective;however,the
success of the mediation component of the program is measured by tracking the
percentage of mediation cases that attain a successful outcome.Catholic Charities defines
a successful outcome as one in which a solution is agreed to by both parties.Catholic
Charities’goal is to reach a successful outcome in 75%of cases.To date,95%of cases
have been mediated successfully.
Success is also measured in both programs by the use of client surveys,which measure
how satisfied clients are with the services they received.Topics addressed in the surveys
include convenience and quickness of services,accessibility of staff,ability to be
connected with needed resources,whether or not clients feel they were treated with
dignity and respect,and whether or not clients would recommend the program to others.

d)In the first 10 months of 2012,Catholic Charities’EA program has assisted 568
unduplicated households with 71 3 units of service.Of these households,46 were in Eagle
County.The CIS program has provided 327 units of service to 121 unduplicated
households in Eagle County.Overall,Catholic Charities has served 167 Eagle County
households in the first 10 months of 2012.
6.COLLABORATION
Catholic Charities collaborates with several agencies as part of its EA and CIS programs.To
completely meet a client’s financial needs,we may collaborate with Eagle County,Salvation
Army,Bright Future Foundation,and others.Many collaborative efforts have evolved out the
CIS program.Catholic Charities spearheaded the Law Enforcement lmmigrant Advisory
Committee,a group focused on improving communication and building trust between area law
enforcement and the immigrant community.As the success of that committee grew,a second
committee,the Immigrant Integration Collaborative,was formed in order to identify barriers and
gaps in service delivery to the immigrant community and find solutions to these issues.This
group is composed of agencies from many fields,including education,health,legal,non-profit,
and youth.
7.VOLUNTEERS
Catholic Charities provides a variety of volunteer opportunities.Most often,volunteers assist
with our front-desk coverage and with client intake.Lawyers provide consultation,pro bono.to
many clients who would not otherwise be able to afford legal services.In the past year,we had
seven volunteers log over I ,000 hours,We are currently recruiting volunteers for family
mentoring and to help us organize and plan a fund-raising event for the spring.

8.PLANNING
Catholic Charities recently developed a three-year strategic plan.An ongoing challenge for the
agency is to maintain and grow revenue.It is especially necessary to grow local support for the
services we provide.Without agency support,otir regional services would not be possible.
However,the executive staff and board of directors need to see the support of the local
community to realize that their efforts in this area are valued.We will continue to clarify our
service priority areas,advocate on behalf of the most vulnerable,and build bridges with various
communities.
