HomeMy WebLinkAboutC16-376 Maple Star ColoradoSECOND AMENDMENT TO AGREEMENT BETWEEN EAGLE COUNTY, COLORADO AND MAPLE STAR COLORADO THIS SECOND AMENDMENT (“Second Amendment”) is effective as of the _____ day of ______________, 2016 by and between Maple Star Colorado, a Colorado non-profit corporation (hereinafter “Contractor” or “Consultant”) and Eagle County, Colorado, a body corporate and politic (hereinafter “County”). RECITALS WHEREAS, County and Consultant entered into an agreement dated the 19th day of November, 2015, for certain Services (the “Original Agreement”); and WHEREAS, by a First Amendment dated May 31, 2016 the parties modified the compensation rate and extended the term through May 31, 2017; and WHEREAS, the parties desire to increase the scope of services and modify the compensation rate as set forth below. SECOND AMENDMENT NOW THEREFORE, in consideration of the foregoing and the mutual rights and obligations as set forth below, the parties agree as follows: 1. The Original Agreement shall be amended to increase the scope of services to include the additional services set forth in Exhibit A-1, attached hereto and incorporated herein by reference. 2. The Budget/Pricing section of Exhibit A of the Original Agreement shall be deleted in its entirety and replaced with the following language: “The compensation for the Services set forth in Exhibit A of the Original Agreement shall not exceed $90 per hour for therapeutic supervised visitation. The compensation for the additional services set forth in Exhibit A-1 herein shall not exceed $900 per moderate level trauma assessment. The maximum contract amount is not to exceed $20,000. There shall be no compensation for travel time or travel related expenses.” 3. Capitalized terms in this Second Amendment will have the same meaning as in the Original Agreement. To the extent that the terms and provisions of the Second Amendment conflict with modify or supplement portions of the Original Agreement, the terms and provisions contained in this Second Amendment shall govern and control the rights and obligations of the parties. Except as expressly altered, modified and changed in this Second Amendment, all terms and provisions of the Original Agreement shall remain in full force and effect, and are hereby ratified and confirmed in all respects as of the date hereof. 5. This Second Amendment shall be binding on the parties hereto, their heirs, executors, successors, and assigns. A Trauma Assessment focuses on the ongoing impact of potentially traumatic events and the ways those events may alter the child's level of functioning. Assessment recommendations focus on the ways to utilize resiliency based strategies to mitigate the impact on the child's future functioning in the three key areas of resiliency: relationships, mastery/efficacy and affect regulation. Trauma Assessments allow Eagle County Department of Human Services (ECDHS) to focus beyond traditional mental health diagnoses, treatments, and behavioral-based interventions. Instead the focus becomes building resiliency through alternative trauma-informed supports that uniquely fit the child’s needs. The ECDHS Trauma Coordinator will facilitate the scheduling of the assessment, gathering and providing client information to the clinician for review prior to meeting with the client and inform client of processes. Scope of Services: Maple Star Colorado will provide Moderate Level Trauma Assessments for eligible clients of Eagle County Department of Human Services (ECDHS). The Trauma Assessment includes the following deliverables: x Pre Assessment ƒ Review of client child abuse/neglect history, educational records/IEP, prior mental health assessments, medical/health information ƒ Ethnographic interviews with current caregiver/ birth parent(s) and caseworker ƒ Review of - Trauma Symptom Checklist for Young Children (TSCYC), Child Behavior Checklist (CBCL), Teacher Report Form (TRF), Sensory Profile Caregiver Questionnaire x Assessment (2-3 hours with child/youth) ƒ Assessment will be completed by one to two clinicians ƒ Clinicians assess verbal and nonverbal cognitive ability with the K-Bit (Kaufman Brief Intelligence Test), quick estimate of intelligence ƒ Clinicians assess language fundamentals with the CELF 5 Language Screener (not the full tool) ƒ Clinicians assess visual motor integration with the Beery VMI ƒ Clinicians assess social communications with the Pragmatics Protocol ƒ Clinicians complete psychosocial interview x Post Assessment ƒ Clinicians provide immediate recommendations in person or by phone to the ECDHS caseworker and caregiver(s) ƒ Clinicians develop written report of assessment ƒ Clinicians debrief with family DocuSign Envelope ID: 75E6B2D3-CEE3-4D9F-AD05-213BEF731C20 2 ƒ Clinicians present recommendations at a Family Engagement Meeting with the ECDHS caseworker, caregiver(s), child/youth, if age-appropriate. Clinicians who complete Trauma Assessments will have completed the two day clinician assessment training with Dr. James Henry, Ph.D., or one of his associates. The clinicians also agree to uphold the requirements of the assessment fidelity endorsement process. All Trauma Assessments must meet the elements of the Quality Assurance (QA) process which is completed on a random basis by the Eagle County Trauma Coordinator. The QA elements are available upon request. Schedule All components of the Trauma Assessment will be completed within six weeks of the date the referral is sent to Maple Star Colorado. The client appointment/trauma assessment will be scheduled within two weeks of the referral date. The written report of assessment will be completed within four weeks of client appointment/trauma assessment. Fees Moderate Level Trauma Assessments are reimbursed at the rate of $900 per assessment. Maple Star Colorado will submit an invoice within 30 days of the completion of the trauma assessment (the trauma assessment is considered complete when the clinician(s) present recommendations at the family engagement meeting).
