Memorandum of Understanding ("MOU")
Between Rocky Mountain Health Plans ("RMHP) and
E Co Government
Accountable Health Communities Model and Vision
Western Colorado has a history of successful community collaboration to promote health. Currently,
several localipartners are engaged in effortsto develop a strong networkthatwill provide more
effective, whole-person support to individuals with multiple health, behavioral and social needs. These
programs include an array of federally-funded initiatives that are focused upon prevention and
integrated behavioral health, such as the Comprehensive Primary Care (CPC) initiative and State
Innovation Models (SlM) Cooperative Agreement. Western Colorado stakeholders are also engaged in
the Colorado Opportunity Project, an effort to improve the social and economic prospects of
Coloradans.
Creating healthy and equitable communities in Western Colorado will require a multipronged effort to
address all of the factors that influence health outcomes - health care, behavioral, environmental, social
and economic factors. Work must be done at the local level, as well at the state and national level - to
speak with one voice regarding the industry, public programs and policies that impact the health of the
people we serve and our entire community,
The Accountable Health Communities Model presents a groundbreaking opportunity to improve
coordination among clinical, behavioral and community service providers. Leaders in several domains,
from public health and human services - to hospitals and health care professionals - share a common
vision for Western Colorado. Participation by community based organizations in this effort will ensure
that the needs of rural and frontier areas are better incorporated in public policy and financing
arrangemenrs.
Communitv Based Organization Activities
lf awarded the Accountable Health Communities Model (AHCM) Program, our organization will
collaborate with RMHP and other Members of the Consortium (all Parties with an MOU with RMHP for
AHCM) to develop strategies to ensure that Medicare, Medicaid and Dually Eligible individuals utilizing
clinical settings who have social needs are connected tothe available communityservices.
Specifically, our organization will participate in the initial planning process and development of referral
design. In addition, our organization will be engaged in ongoing process improvement by being an active
participant in an RMHP supported AHCM Advisory Board.

Through this project, our organization will receive referrals for individuals in the community who have
Medicare andl or Medicaid.
To support quality improvement and program evaluation strategies, our organization will submit
individual-level data into RMHP's AHCM Reporting Solution. Submitted data will include:
o Total number of eligible beneficiaries served
o Services provided
o Cost of services provided
o Referral outcomes
Our organization will also support the Community Navigators as they track service utilization and client
outcomes.

Eagle County Department of Human Services: Child Care Assistance, Child Support Services, Food
Assistance, Energy Assistance, Financial Assistance, Medical Assistance, Veterans Services
Eagle County Department of Public Health & Environment: Family Planning, lmmunizations, WlC, Nurse
Family Partnership, Chronic Disease Self-Management, Falls Prevention, Senior Meals, Services and
Transportation
Eagle County Housing Department: Down Payment Assistance, Low-Cost Housing Placement (based on
availability)
ECO Transit: Eagle County's Public Transportation System provides multi-modal, environmentally
sensitive public transportation choices that are safe, efficient, and reliable. ECO provides bus service 21
hours per day, 7 days per week, 365 days per year, with a total fleet of 31 buses

Prior relationship and collaboration experience with RMHP:
Eagle County Public Health and Rocky Mountain Health Plans work together and are are active board
members of the West Mountain Regional Health Alliance (WMRHA). Through this partnership we work
together to improve access to quality health care.
As a representative of our organization, I acknowledge an understanding that the purpose of the AHCM
model is to "assess whether systematically identifying the health rglated social needs of community-
dvr/elling Medicare and Medicaid beneficiaries, including those whd are dually eligible, and addressing
their identified needs, impacts those community-dwelling beneficiaries total health care costs and their
inpatient and outpatient healthcare utilization" (Accountable Health Communities Initial
Announcement).
Opting Out
The community based organization may elect to opt out of this pilot at any point. lf an organization
decides to opt out, the organization will follow the following procedure, which includes notice of intent
to opt-out to RMHP, 30 days to resolve, and 30 days final written notice to cease participation:
L, Notify the RMHP AHCM Project Manager in writing of a desire to withdraw from the project.
This notification must include the reasons why your organization seeks to withdraw.
2. Participate in meetings and discussions with RMHP representatives and, as appropriate or
relevant, community representatives to see if any solution to challenges can be identified.
3. lf all of the above steps are met and no satisfactory solution can be identified, then the
organization may provide final written notice that in thirty days the organization will cease
participation in the pilot.
