Conlract Name: PURCHASE OF SERVICES CONTRACT TITLE lII OF THE oLDER
AMERICANS ACT And STATE OF COLORADO OLDER COLORADANS
ACT Contract Number 16/17-FY Eaole PHED (herein the "Original Contract")
1) Option to renew only for an additionalterm.
2) In accordance with Section 6 of the OriginalContract between the Northwest Colorado
Councilof Governments and Eagle County, Colorado through its Public Health and
Environment Department, the NWCCOG hereby exercises its option for an additionalterm
beginning Julv 1 . 2016 and ending June 30. 2017 at an amount specified and for the service
levels specified in the approved associated electronic SFY 2015 -2016 Contract Year
documents submitted via the Contract Fleimbursement System.
3) f n accordance wlth Section 7 of the Original Contract the amount of funding remains
unchanged as consideration for services ordered under the contract for the State Fiscal year
2016-2017. The total amount of contract funding including any previous amendments, option
letters, etc. is up to but not exceeding a total of $10'!_.89600
4) The following attachments are hereby incorporated to the Original Contract:
- Option Letter No. 1
- the approved information contained in the electronic submission of the SFY 2015-2016
Contract Year documents.
The recitals mutually agreed to in the PURCHASE OF SERVICES CONTRACT TITLE lll OF
THE OLDER AMERICANS ACT and STATE OF COLORADO OLDER COLORADANS ACT
Contract Number 16/17-FY Eaqle PHED and Exhibits, as amended, shall remain in fu[ force
and effect.
The effective dale of this Option Letter is upon approval of the Executive Director of the
Northwest Colorado Council of Governments.
