2. tlnderstarrdirtg Employee Perspectives: Conduct outreach employees that arc considered
a "vulnerable population" as defined by thc Wo*Well Collaboretive pmjcct team. The
intcnt of the oulrearch will b€ to understatrd how to bcst capture employee perspcctives
and how to litrme questions that are easily understood arrd meaningfirl to omployees
and/or ernployers in other phases of this proj€ct. Ilowthis outreach is condusted will bc
determined by the Conractor and Eagle Courrly $taff. A brief summary report of the
rpsuhs ol'lhe outreach efforts ryill be provided.
3. Focus group/discussion with tJ/orklVell Advisory Cornmhtee on Health Equrty: Plan and
lead a facilitated discussion on health cquity with the WorkWell Collaborative Advisory
Comrnittee to gain their insight about tnirrging a health equity lcns to wurksite wellness
and the WorkWell Collaborativc Project. A brief surnnrary Feport of lhis discussion will
be providad (m+y be cornbined with thc s€trli-structurEd interview rcport).
4. Presenl rtsultsfrom the semi-stnrctured interviews back to the WorkWell Collabomtive
Advisory Cornmittee al a future moeting, preftrably in-persorr (or by phone if nccdcd).
5. Provide guidauce on othcr hcalth equity initiatives by [aglc County Public tlealth and
Environment via phone coruultstion; may rcquire advaloe rcview of provided
docurnenls.
Fccs:
The Coutractor will subrnit an invoice by the 2d of each month. The invoice will include all
costs associated with thc Project as delined above. The cornpensation shall uot cxceed $5,000 for
the Serviccs dcscribed in Exhibit A-1. The hourly ratc paid lo thc Contactor shall tre $125 per
hour.
