SECONDAMENDMENT TO AGREEMENT BETWEEN
EAGLE COUNTY. COLORADO

ALPINE SPRINGS COLTNSELING, P.C.
AMENDMENT ("secondAmendment") is effective u, of tfr. Ld"day of
,2016 by and between Alpine Springs Counseling, P.C. a Colorado professional
corporation (hereinafter "Contractor") and Eagle County, Colorado, a body corporate and politic
(hereinafter "County'').
RECITALS
WHEREAS,County and Consultant entered into an agreement dated the lTthday of March, 2014,
for certain Services (the "Original Agreement"); and
WHEREAS, by the First Amendment dated June 12,2015, the term of the Original Agreement
was extended to May 3I,2016; and
WHEREAS, the term of the Original Agreement expires on the3l'tday of May, 2016 and the
parties desire to extend the term of the Original Agreement for an additional year on the same
terms and conditions as set forth in the Original Agreement; and
WHEREAS, the parties desire to modify the compensation rate as set forth below.
SECOND AMENDMENT
NOW THEREFORE, in consideration of the foregoing and the mutual rights and obligations as
set forth below, the parties agree as follows:
The Original Agreement shall be amended to extend the term to the 3l"day of May,
2017.
The Payment and Fee Schedule in Exhibit A of the Original Agreement is hereby
amended to read as follows:
"County will compensate Contractor for performance of substance abuse treatment
services as follows: (1) Face to face contact with clients for individual outpatient
services - $90 per hour for In Office services in English, $100 per hour for In Office
specialized/bilingual services,$ 100 per hour for In Home/Off Site services in English,
$105 per hour for In Home/Off Site specialized/bilingual services (2) Face to face
contact with clients for group outpatient services - $45 per hour for In Office services
in English, $55 per hour for In Office specialized/bilingual services, $55 per hour for
In Home/Off Site services in English, $60 per hour for In Home/Off Site
specialized/bilingual services (3) Detox services - $140 per day for In Office services
in English, $150 per day for In Office specialized/bilingual services (4) Substance
abuse monitoring services - $10 per episode for Breathalyzer testing, $20 per episode
for urine analysis testing, $35 per episode for ETG analysis.

County will compensate Contractor for performance of domestic violence treatment
services as follows: (1) Face to face contact for domestic violence treatment services -
$85 per hour for In Office services in English, $95 per hour for In Office
specialized/bilingual services, $95 per hour for In Home/Off Site services in English,
$100 per hour for In Home/Off Site specialized/bilingual services (2) Face to face
contact for group domestic violence services - $45 per hour for In Office group
counseling services in English, $55 per hour for In Office specialized/bilingual group
counseling services.
County will compensate Contractor for case management services as follows - $35
per hour.
The maximum contract amount is not to exceed $10,000. There shall be no
compensation for travel time or travel related expenses."
Capitalized terms in this Second Amendment will have the same meaning as in the
Original Agreement. To the extent that the terms and provisions of the Second
Amendment conflict with modify or supplement portions of the Original Agreement,
the terms and provisions contained in this Second Amendment shall govern and
control the rights and obligations of the parties.
Except as expressly altered, modified and changed in this Second Amendment, all
terms and provisions of the Original Agreement shall remain in full force and effect,
and are hereby ratified and confirmed in all respects as of the date hereof
This Second Amendment shall be binding on the parties hereto, their heirs, executors,
successors, and assigns.
IN WITNESS WHEREOF, the parties hereto have executed this Second Amendment to the
Original Agreement the day and year first above written.
