HomeMy WebLinkAboutC16-183 Rocky Mountain Counseling LLCSECOND AMENDMENT TO AGREEMENT BETWEEN EAGLE COUNTY, COLORADO AN} ROCKY MOUNTAIN COLTNSELING, LLC THISJECOND AH}PMENT ("'second Amendment") is effective as of the L{ day of ,2416 by and befween Rocky Mountain counseling, LLC a col,orado limitedliability company, (hereinafter "Contractor" or "Consultanf) and Eag-le County, Colorado, a body corporate and politic ftereinafter .,County'). RECITALS WHEREAS, County and Consultant entered into an agreement dated the 4th day of February,24t5, for certain Services (the "Original Agreement"); and WHEREAS, by the First Amendment dated June 12, 20l5,the term of the Original Agreement was extended to May 31,2016; and WHEREAS, the term of the original Agreement expires on the 3l't day of May, 2016 and theparties desire to extend the term of the Original Agreement for an addiiional y"u, on the same terms and conditions as set forth in the original Agreement; and WHEREAS, the parties desire to modifu the compensation rate as set forth below. SECOND AMENDMENT NOW THEREFORE, in consideration of the foregoing and the mutual rights and obligations as set forth below, the parties agree as follows: l. The Original Agreement shall be amended to extend the term to the 3l*t day of May,2017. €' 2.The PpvFent and 4eq Schedule in Exhibit A of the Original Agreement is hereby amended to read as follows: "County will compensate Contractor for performance of services as follows: (l) Faceto face contact with the ciients for individual outpatient services - $g5 per hour for lnOffice services (2) Face to face cortact with clients for group outpatient services - $45 per hour for In Office services (3) Face to face conta"t for supervised visitationservices - $35 per hour for on-siteloffice services, $45 per hour for off-sitecommunity services (4) Case management services - $35 pei hour. The maximum contract amount is not to exceed $101000, There shall be no compensation for traveltime or travel related expenses.'o Capitalized terms in this Second Amendment will have the same meaning as in the Original Agreement.
