THIS FIRST AMENDMENT ("First Amendment") is effective as of the lJ-day of
{l,qv\ati ,20l6by and between Jessica Waclawski, LCSW a sole proprietoi (hereinafter
"Consultant" or "Contractor") and Eagle County, Colorado, a body corporate and politic
(hereinafter "County'').
RECITALS
WHEREAS, County and Consultant entered into an agreement dated the 5th day of February,
2016, for certain Services (the "Original Agreement"); and
WHEREAS, the Original Agreement contemplated that the Consultant would perform certain
duties with compensation in an amount not to exceed $12,500; and
WHEREAS, County and Consultant desire by this First Amendment to expand the scope of
Services and compensation as set forth in the Original Agreement.
FIRST AMENDMENT
NOW THEREFORE, in consideration of the foregoing and the mutual rights and obligations as
set forth below, the parties agtee as follows:
1. The Original Agreement shall be amended to include additional Services as described
in Exhibit A-1, which is attached hereto and incorporated herein by reference.
2. The compensation for the additional Services set forth in Exhibit A-1 shall not exceed
$5,000 or a total maximum compensation under the Original Agreement and this First
Amendment of $17.500.
Capitalized terms in this First Amendment will have the same meaning as in the
Original Agreement. To the extent that the terms and provisions of the First
Amendment conflict with, modifu or supplement portions of the Original Agreement,
the terms and provisions contained in this First Amendment shall govern and control
the rights and obligations of the parties.
Except as expressly altered, modified and changed in this First Amendment, all terms
and provisions of the Original Agreement shall remain in full force and effect, and are
hereby ratified and confirmed in all respects as of the date hereof.
This First Amendment shall be binding on the parties hereto, their heirs, executors,
suscessors, and assigns.

Scope of Services
Description of Services: Jessica Waclawski, LCSW will provide Mental Health Counseling for Child
Welfare clients including: families, children and parents in her office located at 5l Eagle Road in Eagle-
Vail, CO 81657. These Services shall include, but not be limited to, oversight and development of social-
emotional screening tools, assessments, and service plans, observations of children with social-emotional
concems, providing consultation to families demonstrating mental health concerns, identification of
concems through screens or direct referral, providing staff mental health support both to parents and
families.
Contractor will provide in-person individual, couples, and family therapy intervention specifically:
I. Individuals (age range 14 - 60 years):
i. Support after intensive treatment (such as substance abuse, sexual abuse, or trauma
treatment) to maintain well-being
ii. Women empowefinent: increase self-esteem, healthy boundaries, reduce feelings of
depression or anxiety
iii. Support through various transitions (grief, divorce, moving, career change, birth, etc...)
[. Couples (age range l8-60 years):
i. Improve relationship dlmamics
ii. Support through separation
iii. Support with parenting
III. Families (age range 0-60 years):
i. Assist with transitions (reunification, safety, adolescent changes, blended family, divorce,
new birth, etc.)
IV. Perinatal (pre & post-birth) (age range 14-60 years):
i. Assess for risk factors for postpartum mood disorders in pregnancy
ii. Specific interventions to reduce risk factors
iii. Assess for active mood disorders in postpartum
iv. Specific interventions to reduce symptoms in postpartum, increase healthy attachment,
and provide ongoing assessment
V. Child Social-Emotional Assessment (age range 0-5 years):
i. Assess for child's social-emotional development
ii. Provide individual support plans for targeted interventions
Rate Schedule:
I. Individual: $75 perhour
II. Couple/Family: $75 per hour
III. Child Social-Emotional Assessment: $100 per hour
IV. Perinatal: Individual, Couple, Family rates apply
V. Family Engagement Attendance: $55 per hour
Provider Responsibilitv
I. Provide Eagle County with a treatment plan within 30 days of first appointment.
II. Submit monthly reports identifying dates of visits, progress towards treatment plan goals, and
projected time frames for service delivery with monthly invoice billing.
ru. Submit monthly invoices and progress notes by no later than the 5'of every month for the
previous month of services.
IV. Upon termination of services, provide a final report indicating the client's success in treatment.
V. Attend all Eagle County Family Engagement meetings when providing services for a client.
