to the agreement made effective January 1,2016 by and between Northwest Colorado
Council of Governments, 249 Wanen Ave., P. O. Box 2308, Silverthorne, CO 80499-
2308 and Eagle County, Colorado through its Public Health and Environment
Department, PO Box 660, Eagle, CO 81631 for the Older Americans Act seMces of
Congregate and Home Delivered Meals.
AGREEMENT PERIOD. The period of performance minors the contract, paragraph 6.
NWCCOG will increase the contract amount for Congregate Meals by $2,000 to a total
of $59,229 and will increase the contract amount for Home Delivered Meals by g1,000
to a total of $13,990. This contract increase is for the Minturn Meal site and iniludes the
following unduplicated clients and units of service:
