This Assistance Network Grant Agreement Addendum ( "Addendum ") augments the Assistance
Network Grant Agreement ( "Agreement ") by and between the Colorado Health Benefit
Exchange, doing business as Connect for Health Colorado@ (C4HCO), and Eagle County Health
and Human Services ( "Grantee ") with the Effective Date of 11/1/15. This Addendum is made and
entered into as of 11/1/15 (the "Assistance Network Grant Agreement Addendum Effective
Date ").
This Addendum is a binding contract between the parties, and is hereby made a part of, and
incorporated by reference to, the Agreement. This Addendum will supersede and control over
any contradictory terms set forth in the Agreement with respect to the services set forth herein,
and will be deemed to have augmented and modified the rights and obligations of the parties
under the Agreement to the extent necessary to give each provision of this Addendum full force
and effect. Following this Addendum, all references to the term "Agreement" in the Agreement
will include the terms and conditions of the Agreement and this Addendum, for the term of this
Addendum, as herein defined.
In consideration of the mutual promises in this Addendum, it is the desire of Grantee to add
additional services to the agreement and therefore, the parties agree as follows:
• C4HCO will provide additional funds to Grantee in the amount of $18,034. These funds
are to be provided to Grantee in addition to those in the Approved Budget provision of
the Agreement.
The funds provided by this Addendum are to be used by Grantee only to increase
Grantee's staffing for the period of November 1, 2015 to December 31, 2015 and for
unanticipated increased benefit costs for current staff. The increase in staffing for said
time period is to allow Grantee to assist former and current customers of the Colorado
HealthOp seeking to attain other health insurance coverage through C41-ICO as a result
of the 9olorado HealthOp's dEfflarture from the Colorado health insurance market.
