THIS Memorandum of Understanding("MOU")is made this 21st day of October ,2015,by
and between the members of The Healthy Communities Coalition to support the work of the
American Planning Association and the American Public Health Association's Plan4Health
project made possible through the Centers for Disease Control and Prevention(CDC)as part of
the National Dissemination and Implementation program within the Division of Community
Health,Funding Opportunity Announcement#DP 14-1418.
I. PURPOSE
This agreement is made in order to define the relationship between coalition partners and to
outline the respective roles and responsibilities of each entity related to the Plan4Health grant
agreement with the American Planning Association.
The Healthy Communities Coalition share the primary goal to improve the public's health and
promote equity in health status for all. Parties enter into this agreement understanding that its
provisions will contribute to the capacity of all organizations by strengthening the foundation for
mutual support,accountability and partnership. Coalition members do this through the
collaborative use of expertise,influence, and resources to grow stronger as organizations and to
promote shared priorities in the areas of planning and public health.
II. RESPONSIBILITIES
Please list general membership responsibilities.
• Eagle County Public Health and Environment—Coordinate Coalition activities,
meetings, work groups etc., co-lead the Plan4Health project.
• Eagle County Planning Department—Co-lead the Plan4Health project in the context of
the Healthy Communities Coalition.
• Steering Committee—Comprised of multi-sector professionals to provide oversight and
guidance for Coalition activities, including Plan4Health grant strategies.
• Built Environment Work Group— Will provide guidance for the policy scan process, in
collaboration with subcontractors hired and Towns.
• Healthy Food Access Work Group— Will inform the policy scan process to assure healthy
food access considerations are included in the methodology.
III. TERMS OF UNDERSTANDING
• This MOU is not intended to create any agency or employment relationships between the
parties hereto nor is it intended to create any third party rights or beneficiaries. No Party
shall have authority to bind the other unless expressly agreed in writing.
• The term of this MOU coincides with the Plan4Health grant period,from November 1,
2015 through July 31, 2017.
• Any party to this MOU may terminate this MOU upon thirty(30) days written notice to
the others with or without cause and without penalty or liability.
