Connect for Health Colorado Assistance Network Agreement
This Extension Amendment between Eagle County Health & Human Services, Assistance
Site ("Grantee") and Connect for Health Colorado ("C4HCO") amends the Connect for Health
Assistance Network Grant Agreement ("Agreement")between the parties as set forth below and
shall be effective upon its execution by both parties.
The Agreement shall be amended as set forth below. All other provisions, terms and conditions
within the Agreement or incorporated by reference shall remain in full force and effect.
1. Extension of Term
By operation of law, the 2015 open enrollment period for benefit year 2015 has been changed
and extended to begin November 15, 2014 and continue through February 15, 2015.
Effective as of the execution of this Extension Amendment, Paragraphs 2.A. and 3 of the
Agreement shall be amended to change the grant or award period with respect to the
operations of the Grantee to end June 30, 2015.
2. Grant Funds
The grant period is extended until June 30, 2015, C4HCO will allow carry over funding of a
portion of unspent funds. The total allowed amount for spending from July 1, 2013 to June
30, 2015 shall be $750000. Funding shall be allocated as submitted on the original approved
budget, attached to the original agreement and revised as attached in consideration of the
reduced total budget amount if indicated. Revisions within the budget are permitted in
accordance with paragraph 2.B. of the Agreement. Expenditures under this Extension
Amendment must be made by June 30, 2015 and claimed for reimbursement by July 31,
2015.
3. Grant Extension Scope of Work
The Scope of Work for the Extension Amendment is the Scope of Work described in
Paragraph 1 of the Agreement, as amended by the Amended Scope of Work and Goals and
attached to this Extension Amendment.
4. Grants and other Requirements
Grantee acknowledges that payments made by C4HCO under the Connect for Health
Assistance Network Grant Agreement and this Extension Amendment may be paid out of
funds from the Cooperative Agreement to Support Establishment of State-Operated Health
Insurance Exchanges dated February 22, 2012 between the Centers for Medicare and
Medicaid Services within HHS, and C4HCO (the "Grant"). C4HCO will provide a copy of
the grant to Grantee at its request. The provisions of the Grant required to be included in this

Agreement, and the provisions of federal law required by each Grant to be flowed through as
obligations under the Agreement and this Extension Amendment, and any other provisions of
federal or state Law binding on Grantee because it is paid out of Grant funds, or any laws
applicable because of C4HCO's activities including the operation of a health insurance
exchange, are hereby incorporated into and made a part of the Agreement and this Extension
Amendment, and Grantee will comply with all of such requirements, including E.O. 11246,
"Equal Employment Opportunity," as amended by E.O. 11375, "Amending Executive Order
11246 Relating to Equal Employment Opportunity," and as supplemented by regulations at
41 CFR part 60, "Office of Federal Contract Compliance Programs, Equal Employment
Opportunity, Department of Labor."
