Printed On:29 May 2014 Animal Assistance Foundation Grants 1 0)114C-Vi • FollowUp Animal Assistance Foundation Use of Funds* All of the grant funds are to be used for, and only for, general support of this restricted grant program.Use of any portion of the grant funds, including any interest earned thereon, for any other purpose must have the prior written approval of the Foundation. Chokes I Agree To The Above Regulatory Compliance Cooperation* Grantee will cooperate with AAF in supplying additional information or in complying with any procedures which might be required by any governmental agency in order for AAF to establish the fact that it has observed all requirements of the law with respect to this grant. Chokes I Agree To The Above Grant Reporting* Grantee agrees to furnish scheduled reports on the use of the grant. Grant forms will be provided prior to the end of each reporting period. Reports must be provided on the following basis: •A six (6) month accounting ofexpenditures and grant program status. • A year end accounting of expenditures and grant program status. Chokes I Agree To The Above Financial Records* Grantee agrees to keep its financial and other records so that they adequately show the use of the grant funds exclusively for the grant's purpose. Chokes I Agree To The Above Recognition* Grantee agrees to recognize AAF for this contribution in written and broadcast media in ways appropriate for this level of contribution and general acknowledgment with other funders. Chokes I Agree To The Above Asilomar Accords* AAF ascribes to the Asilomar Accords preferring terminology and policies that are clear, positive in message, and build understanding between shelters and with the public such as "open" and "limited admission" (as opposed to "no kill"). Choices I Agree To The Above Animal Behavior Training* Animal Assistance Foundation requires all grant recipients to embrace positive reinforcement methods for any training and behavior modification programs. As such we prohibit the following: • The use of any shock, choke or prong collar in any training activity • The use of alpha roll, muzzle hold, scruff shaking, ear or paw pinching,tongue holding, helicoptering, choking, hanging, stringing up or any other method that relies on physical or verbal intimidation, fear or pain • Squirt bottles or loud noise aversives Choices I Agree To The Above Community Commitment* AAF is committed to good communication among organizations and grantees. Your communication to and about other animal welfare organizations and within your community should always reflect the principals of honesty, fairness and respect. Choices I Agree To The Above Veterinary Care Standards* If applicable, grantee agrees to utilize the highest standard in analgesia anesthesia and surgical protocols.By signing this letter youconfirm that you endeavor to adhere to the standards of care for anesthesia asoutlined in the AAHA Anesthesia Guidelines for Dogs and Cats and for pain management as outlined in the AAHA/AAFP Pain Management Guidelines for Dogs& Cats. If your protocol for analgesia anesthesia or pain management varies from these guidelines, please provide a response outlining any alternative protocol employed by your clinic. Choices I Agree To The Above AAF Name & Logo* By accepting this grant the recipient agrees that AAF may at any time restrict the use of the AAF name and logo upon AAF's discretion.
