Section --- Project Summary
Project Summary:
Include a brief description of how this project will address the 2013 JAG goals. Also include
target population, estimated number to be served annually and cost per person (if applicable).
See instructions for futher information.
Ea le County's overarching goal is to create a system of care for serving
children and youth identified as at-risk of entering the child welfare and/or
juvenile justice system. This county-wide collaborative project aims to improve
access to care and outcomes of treatment for adolescents with multi-systems
needs including but not limited to: mental health treatment, substance abuse
trealtment, juvenile justice, child welfare, and education.
In 21012, Eagle County received Colorado Justice Assistance funds to further
these efforts. With this funding Eagle County Wayfinder is able to complete two
major goals; hire a 2nd case manager in effort to provide Wayfinder services
county wide for 100 youth in 2012-2013 and provide bilingual therapeutic
counseling services. Eagle County is requesting 2013 Colorado Justice
Assistance Grant funds, to further integrate and develop Wayfinder as a system
of care in Eagle County and surrounding partnering counties in order to serve
120 youth.

Project: 2013 Eagle County System of Care for Youth 2013-DJ-13-001508-03-2
Eagle County youth aged 10-18 years and their families are challenged with
increasing levels of substance abuse and mental, emotional and/or behavioral
(MEB) issues. Further, they have limited options for services, particularly ones
that are culturally responsive to the needs of youth of Latino/Hispanic descent.
Eagle County is requesting funds to reinforce current efforts and expand
services to address this growing problem.
There are 3,300 adolescent ages 10 to 18 residing in the county, representing
6% of the total population. Eagle County Schools (ECS) reports a 51% Hispanic
enrollment. For over ten years the county has assessed the needs of
adolescents through a biannual Healthy Kids Colorado survey. The 2011-12 -
survey indicates continued high levels of mental illness and substance abuse in
Eagle County youth. The results demonstrate 22% of ECS students felt "sad or
hopeless for more than 2 weeks" and 11% seriously considered attempting
suicide. In terms of substance use and abuse, 31% of youth reported drinking
alcohol, 18 % of youth report binge drinking and 7 % report using alcohol more
tha 100 days or more. 4-14% reported using drugs, including cigarettes
(13 0); marijuana (28%); cocaine (11%); inhalants (12%); heroin (4%);
met amphetamine (4%); ecstasy (9%); and prescription drugs (14%).
The 5th Judicial District continues to serve higher levels of sentenced and pre-
adjUdicated youth. In FY 2011-2012, 39 youth completed sentencing
requirements, 79% completed the sentence successfully. It is projected that the
nuniber of sentenced youth will remain the same in 2012-2013. Youth
commitment levels have increased 50% in 2012-2013 in the 5th Judicial District.
Pre;adjudicated youth continue to have successful outcomes. Forty two youth
were served in FY 2011-2012 and it is estimated that this amount will remain
steady in FY 2012-2013. Ninety five percent of the pre-adjudicated youth
completed services successfully.
Eagle County Children, Family and Adult Services received an estimated 60
referrals regarding at risk youth in 2012. In addition, Eagle County involved 19
youth and their families in protective services in 2012. Of these 19 youth, 6
were placed in institutions due to complex mental health problems, behavioral
disorders, juvenile justice and/or family dynamic issues. Eagle County Children
and Family Services receives a minimum of 3 requests each year for placement
evaluations for youth at risk of entering the juvenile justice system.
In 2 10, a county-wide needs/gap analysis of existing evidence-based practices
and programs indicated that there was a lack of integration of resources
ded Gated to identifying and planning prevention, intervention and treatment
se ices for adolescent whose risk behaviors exceed state and national levels.
As result, collaborating organizations committed to the development and
impllementation of the system of care. ECS has developed and implemented an
early warning indicator system (EWIS) that will serve as the foundation for early
intervention for adolescents exhibiting risk for drop-out, truancy, delinquency,
and(challenges resulting from MEB issues. As of October 15, 2012, 124 middle
and(high school students in Eagle County School District have been identified
as a`t-risk by the EWIS. Analysis of the data obtained from the EWIS report
indicates that approximately 75% of the identified at-risk students are Hispanic.
Access to care issues faced by the Latino community include: gaps in critical
bilingual services, lack of awareness of available services, lack of process
knowledge for accessing services, and the perceived lack of safety in accessing
seMces.

Project: 2013 Eagle County System of Care for Youth 2013-DJ-13-001508-03-2
Wayfinder System of Care, served 11 youth in 2012 with case management
services. Linking youth and families with beneficial resources was a key effort of
the case managers. These youth and their families needed assistance in
obtaining education, mental health, substance abuse treatment, probation,
economic supportive services and non-profit programming resources.
Youth and their families have limited resources to turn to for addressing the
needs of adolescents in Eagle County, particularly those of Spanish speaking
families and individuals. Currently, there is no bicultural, bilingual mental health
provider in the county. In addition, there are limited mental health providers who
specialize on youth mental health or youth substance abuse. There is no day
treatment or specialized outpatient substance abuse treatment options available
for local youth. The high levels of youth risk, combined with the lack of available
mental health resources to address them, warrant additional collaborative focus
to provide essential and integrated prevention, diagnostic and treatment
services that will reduce the number of incidents occurring at school, and the
number of truancy and criminal cases on the juvenile docket in EC Courts.

Project: 2013 Eaigle County System of Care for Youth 2013-DJ-13-001508-03-2
Eagle County's overarching goal is to create a system of care for serving youth
identified as at-risk of entering the child welfare and/or juvenile justice system.
In an effort to reach this goal Eagle County is requesting $106,716 to complete
the following objectives: 1) Further integrate and develop the Wayfinder System
of Care in Eagle County and support Pitkin County to expand a model of
Wayfinder services in the Roaring Fork River Valley 2) Expand case
management services to serve 120 youth throughout the region 3) Build
infrastructure for direct bilingual, bicultural counseling services for youth and
their families. This project aligns with the Justice Assistance Grant Program
Goal 1, to improve outcomes through effective and efficient use of resources to
bridlge gaps within the criminal and juvenile justice system by aligning youth-
serV'ing agencies, and leveraging services and resources. Secondarily, this
project addresses Goal 2 of preventing and reducing crime and delinquency
through the use of collaborative evidence-based and promising practices, by
enspring that at-risk youth are identified early and receive needed integrated •
collaborative direct services.
With the 2012 Juvenile Assistance Grant funds, the Wayfinder System of Care
was able to focus on three promising practices that will be utilized by Wayfinder
including the utilization of the Early Warning Indicator System throughout local
schools and the implementation of Team Decision Making services. Eagle
County has also identified the need to explore Promotora services to help
enhance the accessibility of Wayfinder System of Care. Using the early warning
indicator system (EWIS),124 kids were identified as at-risk for school drop-out
and multi-system involvement and needing further assessment and 11 youth
received case management services in 2012. The Wayfinder System of Care is
working to train community partners including, private schools, mental health
agehcies, youth serving non-profits and the school system and community
parthers in the El Jebel/ Basalt area to utilize the early warning indicator
database system so that additional youth can be identified. Trainings have been
completed with 10 agencies on Wayfinder and the screening tools since the
beginning of 2012 Juvenile Assistance Grant. Other accomplishments for
Wayfinder in 2012 include: A strategic plan was developed in collaboration with
partnering agencies, including a mission, vision, and short and long goals; The
cas manager job description was written, training protocols adopted and one
cas manager was hired; Wayfinder engaged family partners through
co munity forums and developed the family partner job description and
inv lved local youth through a focus group for logo selection and created the
plan for ongoing youth engagement; Enhanced Cultural and Linguistic
Co petence through written surveys in partnership with Catholic Charities and
a c mmunity focus group that looked at health, education, and cultural
nav gation in Latino Communities; and secured a Spanish- speaking family
parthers.
In 2013, Eagle County will continue the integration and development of the
Wayfinder System of Care. Eagle County will work to increase community
awareness about the needs of youth and how to access the Wayfinder System
of Care for youth. It is a goal to expand the System of Care and case
management services to Pitkin County and El Jebel/ Basalt community
parthers. Eagle County is currently partnering with Pitkin County to provide
regional child and youth welfare services in the El Jebel-Basalt area of the
county. In efforts to continue regionalizing children and youth services in the
Roaring Fork River Valley, Eagle and Pitkin County are collaborating to launch
the Wayfinder System of Care model in Pitkin County. Pitkin County has

recently received community funds to expand services for youth and will be
purchasing Wayfinder case management services. With this increased
integration and collaboration, Wayfinder will serve at minimum 120 youth in FY
2013-2014.
It is also a goal for Wayfinder to fully engage the juvenile justice system by
increasing communication, coordination and utilization of Senate Bill 94
services for youth and providing diversion services for 15 appropriate youth.
The Wayfinder System of Care aims to identify all at-risk youth earlier before
multiple agencies are already involved. Upon identification, specified staff in
partner agencies will perform a quick screen to determine if multi-agency
services are needed. If so, the youth is referred to Wayfinder. Once a referral is
received, a Wayfinder Case Manager will perform a comprehensive assessment
to determine the needs of the youth, coordinate the engagement of appropriate
service provider agencies and work with representatives of those agencies and
the youth and f
mil through Team Decision Making meetings to develop the
long-term treatment plan. The Case Managers then evaluate the progress of the
youth in changing the targeted at-risk behavior. It is anticipated that each
Wayfinder Case Manager will serve 50 youth/families per year. Along with
identifying youth, Wayfinder plans to continue identifying promising and
evidence-based practices to better serve and treat at-risk youth. This work also
includes contracting for direct bilingual counseling and therapeutic services to
support youth and the families involved in Wayfinder.

Thus far, costs to develop and implement Wayfinder and CMP have been
directly funded by Eagle County, Eagle County Schools, the Anschutz
Foundation, the Eagle River Youth Coalition, Vail Police Department, and 5th
Judicial District Probation. Additionally, in-kind support from the 13 partnering
agencies formally included in the MOU indicates the commitment to the ongoing
success of this project.
Eagle County received a total of$100,000 in grant awards as part of the
Communities of Excellence 2012 & 2013 Projects which is supporting further
expansion and integration of the System of Care. Eagle County Collaborative
Management Program received $47,675 in incentive funds that will continue to
assist with project administration and operations.
Additional funds are needed to expand current levels of case management and
to regionalize program services. Additional funds can assist in paying for direct
services for those who do not qualify for assistance or do not have insurance.
Though initial funds for specific needs are sought through this JAG proposal,
long-term sustainability of the positions, the delivery of the services, and the
ongoing administration of the project are being addressed by members of the
Interagency Oversight Group (IOG) that governs the CMP. Further, partners

On oing evaluation will determine the effectiveness of the program and ensure
continuous quality improvement in services provided. Eagle County will focus
on three levels of evaluation: Client progress, service delivery processes, and
overall system efficiency. At the client level, evaluation will include successful
achievement of individual treatment plans; development and utilization of
positive behavioral skills; and improved school attendance and academic
pe rmance. At the direct service level, measures include reduction in the lag
time between an individual's assessment and receipt of services; improvement
in the quality of services; and improvement in the delivery of comprehensive
services.
At the Wayfinder System of Care level, evaluation includes increased access to
appropriate services by the target population, an increase in the number of .
youth who receive intervention services; improved communication among
parther agencies, and a reduction in the cost of direct services. The Project
Director will collect and analyze data from the early warning indicator systems,
the quick screen, and the referral process within each partnering agency. In
addition, data from other sources including child welfare and the judicial system
case service evaluations will be gathered. Data will be evaluated to determine
the efficiency of the system, and to identify service gaps, develop strategies to
address them, and improve the overall quality of direct services. As a state
CMP, the local program will also be evaluated in four domains including juvenile
justiice, child-welfare, education, and mental/public health.
The single point of entry database meets all HIPAA and FRPPA requirements
and is consistently upgraded to ensure that the project continues to meet those
requirements. Consent of the youth will be required before services are
provided and parents will be required to sign "consent to assess and treat" and
"information sharing" consent forms. The SPE database consists of both
evaluative information on each case and serves as a case management tool
designed to improve efficiency in assessment, planning and evaluation.
