( "Amendment ") is made and entered into this % day of

( "County ") and Venturoni Surveys and Research, Inc., a Colorado corporation with its principal
place of business at 422 Idlewild Dr., Dillon, CO 80435 ( "Contractor ")
RECITALS
WHEREAS, County and Contractor entered into an agreement, dated February 12, 2013 (Eagle
County Contract Number 13 -052), for the consulting services ( "Original Agreement "); and
WHEREAS, County and Contractor wish to extend the term and amend the Scope of Work in
the Original Agreement.
NOW, THEREFORE, County and Contractor agree that the Original Agreement will be
amended as follows:
1. Article I, Services Provided, shall be deleted in its entirety and replaced by the following
language:
The Contractor will provide all services, labor, personnel and materials to perform
and complete the services set forth in County's Request for Bids, attached to the
Original Agreement as Exhibit A, and Contractor's Project Narrative attached
hereto as Exhibit B 1 and incorporated herein by this reference (hereinafter called
the "Services" or "Consulting Services ").
2. Section 2.1 of Article II, Term of Agreement, shall be amended to extend the term of the
Agreement, on the same terms and conditions as the Original Agreement, through June 30, 2014.
The Agreement may be renewed for an additional one year term by written amendment to this
Agreement, signed by both parties. The remainder of Article II shall remain unchanged.
3. Section 4.1 of Article IV, Remuneration, shall be deleted in its entirety and replaced with
the following language:
For the Consulting Services provided, County shall pay Contractor at the rate of $85 per
hour. Contractor shall submit monthly invoices identifying the Contracting Services performed
no later than the fifth working day of the following month. The total compensation due
Contractor for the period ending June 30, 2014 shall not exceed $11,560. The full contract
amount from commencement of the Original Agreement through June 30, 2015 shall not exceed
$31,000. Contractor will not be entitled to bill at overtime and/or double time rates for work
done outside of normal business hours unless specifically authorized to do so by County. Fees
for any additional services will be as set forth in an executed addendum between the parties.
Fees will be paid within thirty (30) days of receipt of a proper and accurate invoice from

Contractor respecting Contracting Services. The invoice shall include a description of services
performed. Upon request, Contractor shall provide County with such other supporting
information as County may request.
The remainder of Article IV shall remain unchanged.
4. Section 9.1(a) of Article 9, Notices, shall be deleted in its entirety and replaced with the
following language:

5. County and Contractor agree that, except as expressly altered, modified and changed in
this Amendment, all terms and provisions of the Original Agreement shall remain in full force
and effect during the modified term, and hereby are ratified and confirmed in all respects as of
the date hereof.
6. This Amendment shall be binding on the. parties hereto, their heirs, executors, successors,
and assigns.

Project Narrative
The Venturoni Surveys & Research, Inc. team has extensive experience in project evaluation.
We are knowledgeable about designing effective models for the collection of data. It is
important to build evaluation tools into the design of the project to accurately measure and
quantify project outcomes. We utilize both quantitative and qualitative methodologies to gather
data, and have software systems to design and analyze Internet and paper surveys. Through
extensive experience with focus groups, we have incorporated keypad voting techniques to add
a quantitative aspect to a form of data collection that is essentially qualitative in nature.
The Venturoni Surveys & Research team will provide the following services during the three -
year project:
• Develop and conduct a process and impact evaluation for the Soy Sano Project.
• Attend planning meetings with Latino health leaders, ECPH and other
stakeholders to define culturally appropriate evaluation tools and methods for
tool dissemination.
• Create a Soy Sano evaluation logic model for the scope of the Project.
•. Create and disseminate culturally appropriate evaluation tools.
• Produce annual Soy Sano Project evaluation reports.
For Year One of this project we propose the following:
1. Design the evaluation model for the three year project:
a. Meet with the stakeholder groups to discuss the goals and objectives for each of
the components:
L Nutrition Education
ii. Nutrition Awareness in the Media
iii. Healthy Food Access
iv. Healthy Food Access Policy
b. Through discussions with the stakeholder groups and experts in the field,
determine:

L What are we trying to accomplish?
ii. How can we measure it?
iii. What is the best timeline for taking the measurements? (i.e., pre- and
post- testing)
EXHIBIT
f
iv. How do we define the sample group? (i.e., who is included in the group
to be measured and how do we reach them ?)
v. What are the best methodologies to utilize? (i.e., surveys, focus groups,
etc.)
2. Establish a timeline for accomplishing the program evaluation:
a. Pre - testing (establishing the baseline)
b. Post - testing (measuring changes related to the program initiatives)
c. Data Analysis
d. Production of annual evaluation reports
Costs
The cost to complete this project evaluation for the first year is $9,000. Costs will vary for the
second and third year of the project depending on the design of the evaluation model. In
general, internet surveys can be accomplished at a relatively low cost, paper surveys are more
expensive, and focus groups can be costly too. It may be possible to offset some costs by hand
distribution of paper surveys and using volunteers to enter the data. There may be ways to
offset focus group costs by using volunteers to recruit participants. We anticipate that years 2
and 3 will take more time because more of the project .evaluation measurements will occur in
these years. The figures below for years 2 and 3 are estimates at this point.
