Dear Ms. Koenemann:
This letter is to inform you of the approval of the Eagle County Public Health's mini -
grant application in the amount up to but not more than $3,0.00 for the Behavioral Risk
Factor Surveillance System survey for the mid - valley region..
This is a cost - reimbursement award. The reimbursement to Eagle County Public Health
will be done in two parts:
1. Once we receive your completed invoice and copy of the survey tool for the
mailing of the survey we will send you a check for 90 percent of the awarded
amount ($2,700)
2. the remaining 10 percent of the funds ($300) will be sent at the end of your
project after a presentation of the survey results to the Region 12 Regional
Advisory Council is completed.
The mini -grant program is funded by the Older Coloradans Act -State Funds for Senior
Services, and is administered by the Region 12 Regional Advisory Council of the
NWCCOG - Alpine Area Agency on Aging,
As a recipient of funds from this mini -grant you must comply with this letter of
agreement. The requirements are minimal. Please read, sign, and return a copy of this
letter to the. NWCCOG - Alpine Area Agency on Aging, PO Box 2308, Silverthorne, CO
80498 or by FAX to (970) 468 -1208.
REQUIREMENTS OF SFY'13 MINI GRANT:
1. As the staff person Managing this mini -grant please keep a separate file on it in the
event that NWCCOG - Alpine Area Agency on Aging is audited. If we are audited on this
project you might need to present your file for inspection. The file should be retained for five
years. We suggest naming the file "Mini Grant Award, SFSS SFY'13" and the `name of the
project'.
2. Expenses charged to the project may not be incurred prior to May 1, 2013. Please
keep an accurate accounting of expenses charged to the project in the file, including any
local cash or in- kind contributions for expenses not identified in the application.

3. The survey mailing for your project must be completed no later than June 30,
2013.
In order to receive the 90 percent payment for the project you must submit the following,
items within fifteen days (15) of the awarded project completion: July15, 2013:
NI an invoice for reimbursement for the awarded project [the survey mailing] to the Alpine
Area Agency on Aging;
please include a copy of the BRFSS survey tool.
N/ a one- to two -page summary of the process to date. Please address in the project report
address how you plan to share the data collected with the Alpine Area Agency on Aging
and the county councils on aging for Eagle, and Pitkin counties.

We will not guarantee that you will be reimbursed the 90 percent for the project if the

Acceptance of the SFY'13 Mini -Grant and grant requirements are indicated by your
signature.
