Re: Agreement between Eagle County and Valley View Hospital Association for Medical
Officer Services
Dear Al:
As you know, by contract dated January 24, 2013, Eagle County and Valley View Hospital Association have
contracted for the services of Valley View employees, Dr. Drew Warner M.D. and Dr. Angela Ammon, with Dr.
Warner serving as the Eagle County Medical Officer (the "Agreement "). A copy of the Agreement is enclosed
here for your reference. Section I of the Agreement provides for Dr. Ammon's service as Deputy Eagle County
Medical Officer in Dr. Warner's absence. It has come to my attention that effective February 28, 2013, Dr.
Warner will no longer be an employee of Valley View Hospital. This purpose of this letter is to confirm that Dr.
Ammon will serve as the Eagle County Medical Officer from March 1, 2013 until further order of the Board. It is
our understanding that Dr. Ammon is agreeable to serve as Medical Officer on a temporary basis, up to four (4)
months and will provide all services contemplated by the Agreement during that time. It is understood that Dr.
Ammon will not be available April 4 -7 and April 12 -21. Meanwhile, our Public Health Department is working
towards finding a new arrangement will keep you apprised of any changes that will affect our current Agreement.
If you are in agreement, please countersign this letter in the space provided below and return one executed copy
of this letter to my attention at the address listed above.
We are pleased to continue working with you and your staff and thank you for your on -going partnership and
valuable work in the community.
