BETWEEN EAGLE COUNTY
AND VVMC DIVERSIFIED SERVICES
dba EAGLE CARE MEDICAL CLINIC AND NORTHSTAR UROLOGY
FOR THE PROVISION OF FAMILY PLANNING HEALTH CARE SERVICES
THIS FIRST AMENDMENT ( "Amendment ") is made and entered into this day
of , 2012, between Eagle County, by and through its Board of County Commissioners
(hereinafter referred to as "County ") with a mailing address of 500 Broadway, Post Office Box
850, Eagle, CO 81631 and VVMC Diversified Services dba Eagle Care Medical Clinic with a
mailing address of P.O. Box 40,000, Vail, CO 81658 ( "Contractor ").
WHEREAS, County and Contractor entered into an Agreement dated February 27, 2012
( "Original Agreement ") C12-085; and
WHEREAS, County and Contractor wish to extend the term of the Original Agreement.
NOW, THEREFORE, County and Contractor agree that the Original Agreement is
hereby amended as follows:
1. The term of the Original Agreement is extended to December 31, 2012.
2. All terms and provisions of the Original Agreement shall remain in full force and
effect during the modified term.
3. This Amendment shall be binding on the parties hereto, their heirs, executors,
successors, and assigns.
