HomeMy WebLinkAboutC11-298 211 Service Agreement 2 -1 -1 Service Agreement The following terms of service (the "Agreement ") are effective June 1, 2011. Between: Mesa County Department of Human Services P.O. Box 20,000 -5035 Grand Junction, CO 81502 AND Eagle County Health and Human Services Children & Family Services Department 551 Broadway, P.O. Box 660 Eagle, CO 81631 WHEREAS, actions of the Federal Communications Commission and the Colorado Public Utilities Commission approved use of the three digit telephone number "2 -1 -1" for access to community health and human services information and referral; and WHEREAS, Eagle County Department of Health and Human Services ( "Eagle HHS ") wishes to provide individuals and families in Eagle County with referral information regarding non - emergency services such as food assistance, shelter, job assistance, and low cost health care; and WHEREAS, Mesa County Department of Human Services ( "Mesa HS ") wishes to cooperate with Eagle HHS to provide the 2 -1 -1 service described herein; and WHEREAS, cooperation between Mesa HS and Eagle HHS will be in the general interest and for the general welfare of the residents of Eagle County including individuals and families meeting public assistance eligibility criteria. NOW, THEREFORE, for and in consideration of the agreements set forth herein, Eagle HHS and Mesa HS agree as follows: 1. Scope of Services. To maintain the 2 -1 -1 system in Eagle County, Mesa HS shall provide and/or coordinate agreed upon services that include, but are not limited to the following: • Continue development of an inventory of public agencies/ programs currently providing health and human services in Eagle County. • Maintain a method for regular communication with the health and human service agencies and programs in Eagle County to maintain accurate data within the system. • Integrate and input Eagle County data into the master 2 -1 -1 database administered by Mesa HS. • Provide training and orientation for call specialists related to knowledge or directions, landmarks and other unique information that may be helpful in matching services in Eagle County with callers' needs. • Provide coverage of the 2 -1 -1 Area Information Center (Call Center) with at least one AIRS — Alliance of Information and Referral Systems — certified professional call specialist Monday through Friday, 8:OOam to 5:00 pm. • Submit regular reports to HHS regarding the use of the 2 -1 -1 Area Information Center (Call Center) by Eagle County residents. • Develop and implement a strategy for outreach and promotion of the 2 -1 -1 Area Information Center (Call Center) in Eagle County. 2. Time of Performance. The term of this Agreement shall begin on the 1 day of June, 2011 and shall terminate the 31 day of May, 2012. This Agreement will terminate on the date set forth above, unless renewed by a written extension amendment for another like term, except that any terms or condition may be reviewed and renegotiated annually. 3. Compensation and Method of Payment. Payment will be in one lump sum of $6,225 to the Mesa County Department of Human Services fiscal agent, at the address listed at the beginning of this Agreement. Payment will be applied as described in Exhibit A. 4. Records Reports and Information. At such times and in such forms as may be required, Mesa HS shall furnish statements, records, reports, data, and information pertaining to matters covered by this Agreement. 5. Independent Contractor. Mesa shall perform duties hereunder as an independent contractor and not as an employee of Eagle County or Eagle HHS. No employee, agent, officer, or volunteer of Mesa HS shall be deemed an employee of Eagle County or Eagle HHS. Mesa HS will secure all personnel and materials required to perform the services covered in this Agreement. 6. Termination for Cause. If, through any cause, Mesa HS shall fail to fulfill, in a timely and proper manner, its obligations under this agreement, Eagle HHS shall thereupon have the right to terminate this Agreement for cause by giving written notice to Mesa HS of its intent to terminate and at least ten (10) days opportunity to cure the default or show cause why termination is otherwise not appropriate. In the event of early termination of this Agreement, any payment made by Eagle HHS that Eagle HHS reasonably determines was improper because the services for which the payment was made were not performed as prescribed by the provisions of this Agreement, then upon written notification of such determination and request for reimbursement from Eagle HHS, Mesa HS shall forthwith return such payment to Eagle HHS.
