HomeMy WebLinkAboutC10-219 Western Colorado 2-1-1Service Agreement The following terms of service (the "Agreement") are effective (June 1, 2010) Western Colorado 2-1-1 Between: C/O United Way of Mesa County P.O. Box 153, Grand Junction, CO 81502 AND Eagle County Department of Health and Human Services Children & Family Services 551 Broadway, P.O. Box 660, Eagle, CO 81631 WHEREAS, actions of the Federal Communications Commission and the Colorado Public Utilities Commission approved use of the three digit telephone number " 2-1-1" for access to community health and human services information and referral; and WHEREAS, Eagle County Department of Health and Human Services ("HHS") wishes to provide individuals and families in Eagle County with referral information regarding non-emergency services such as food assistance, shelter, job assistance, and low cost health care; and WHEREAS, Western Colorado 2-1-1 wishes to cooperate with HHS to provide the 2-1-1 service described herein; and WHEREAS, cooperation between Western Colorado 2-1-1 and HHS will be in the general interest and for the general welfare of the residents of Eagle county including individuals and families meeting public assistance eligibility criteria. NOW, THEREFORE, for and in consideration of the agreements set forth herein, the HHS and Western Colorado 2-1-1 agree as follows: Scope of Services. To establish and maintain the 2-1-1 system in Eagle County, Western Colorado 2-1-1 shall provide and/ or coordinate agreed upon services that include, but are not limited to the following: • Develop an inventory of public agencies/ programs currently providing health and human services in Eagle County. Establish and maintain a method for regularly communicating with the health and human service agencies and programs for accurate data within the system. • Integrate and input the data into the masker database for Western Colorado 2-1-1. ~(~~ ?~~/ • Provide training and orientation for call specialists related to knowledge of directions, landmarks and other unique information that may be helpful to matching services in Eagle County with caller's needs. • Provide coverage of the 2-1-1 Area Information Center (Call Center) with at least one AIRS - Alliance of Information and Referral Systems -certified professional call specialist Monday through Friday, 8:00 am to 5:00 pm. • Submit regular reports regarding the use of Western. Colorado 2-1-1 by Eagle County residents. • Develop and implement a strategy for outreach and promotion of Western Colorado 2-1-1 in Eagle County. 2. Time of Performance. The term of this Agreement shall begin on the 1St day of June, 2010 and shall terminate on the 31St day of May, 2011. This Agreement will terminate on the date set forth above, unless renewed by a written extension amendment for another like term, except that any terms or conditions may be reviewed and renegotiated annually. 3. Compensation and Method of Payment. Payment will be in one lump sum of $8,030 to the Western Colorado 2-1-1 fiscal agent, United Way of Mesa County at the address listed at the beginning of this Agreement. Payment will be applied as described in Exhibit A. 4. Records Reports and Information. At such times and in such forms as may be required, Western Colorado 2-1-1 shall furnish statements, records, reports, data and information pertaining to matters coved by this Agreement. Independent Contractor. Western Colorado 2-1-1 shall perform duties hereunder as an independent contractor and not as an employee of Eagle County or HHS. No employee, agent, officer or volunteer of Western Colorado 2-1-1-shall be deemed an employee of Eagle County or HHS. Western Colorado 2-1-1 will secure all personnel and materials required to perform the services covered in this Agreement. 6. Termination for Cause. If, through any cause, Western Colorado 2-1-1 shall fail to fulfill, in a timely and proper manner, its obligations under this Agreement, the Department of Health and Human Services shall thereupon have the right to terminate this Agreement for cause by giving written notice to Western Colorado 2-1-1of its intent to terminate and at least ten (10) days opportunity to cure the default or show cause why termination is otherwise not appropriate. In the event of early termination of this Agreement, any payment made by HHS that HHS reasonably determines was improper because the services for which the payment was made were not performed as prescribed by the provisions of this Agreement, then upon written notification of such determination and request for reimbursement from HHS, Western Colorado 2-1-1 shall forthwith return such payment to HHS. Schedule: The program period will start upon execution of the contract and shall terminate May 31st, 2011. 10) Financial considerations: The contract is for the amount of $8,030 and is fully ~~ budgeted in the 2010 BrightStart budget under line item: 1001-508-6650-89. ~~`~ There are some first year, one-time costs in the contract that will not be recurring in the future.
