Eagle County is applying for $6,000 from United Way of Eagle River Valley to continue the
Home Connections housekeeping program for eight older adults in Eagle County. In the last
year, Home Connections served five older adults with free housekeeping twice a month. The
$6,000 funding from the United Way of Eagle River Valley will allow Home Connections to
help eight clients in 2010-2011.
For the last two years, the United Way of Eagle River Valley has funded Eagle County to assist a
total of eight older adults in Eagle County Home Connections. While this program remains small
in size, it's personal impact is large. The clients currently receiving this service are each at
imminent risk of out of placement in an assisted living facility. Home Connections along with
the other Aging Services programs are the safety net allowing these clients, many who have
spent their entire adult lives here and have children and grandchildren in the area, to remain
safely in our community.
Eagle County Aging Services in collaboration with the Alpine Area Agency on Aging supports a
system of programs that allows older adults to remain safely and independently in our
community. Aging Services is dedicated to serving those over 60 with nutrition, transportation,
and social activities that help to maintain the independence of older adults. In efforts to reach this
goal: Eagle County Aging Services provides 10,000 meals a year to older adults at nutrition sites
in Minturn, El Jebel and Eagle and with home delivered meals for those who are home bound
due to illness and other physical and mental incapacities. Our transportation drivers give 600
rides a month to lunch sites, medical appointments, shopping, errands and the post office.
Volunteers recruited through the Volunteer Center and RSVP Program, provide 150 trips a year
for local adults to medical appointments in county and as far as Grand Junction and Denver.
Home Connections is the newest addition to the Aging Services options.
It is a goal of Home Connections to prevent accidents and injuries of the older adults, as well as

the involvement of more costly adult protection services and the entry into an assisted living
facility. Since it's inception, Home Connections has assisted in preventing the placement of
seven out of eight older adults despite multiple illnesses and ongoing incapacities, only one
client has moved on to an assistive living facility.

PART II APPLICATION FOR PROGRAM FUNDING
1. Describe the program to be funded. If this is a collaborative, interagency project, please
describe each agency's role in the program. Please describe how the need for the program
was determined.
Home Connections will grant free housekeeping services to older adults living in Eagle County
who live at or below 250% of poverty and demonstrate difficulty maintaining activities of daily
living due to either physical or mental health incapacities.
The Aging Services case manager takes referrals on any older adult who is in need of resources
and support to live more safely in their home. The case manager meets with each client who is
interested in and maybe eligible for Home Connections and completes both the Activities of
Daily Living and Individual Activities of Daily Living Assessment scales. In order to be eligible
to receive Home Connections services, the older adults need to have difficulty completing at
least 50% of the tasks on the assessment scales and also meet income eligibility requirements.
The case manager then works with the Home Connections coordinator to decide who to enroll in
the program.
Enrolled clients will receive the Home Connections cleaning services twice a month for two
hours a session. The assistance will include many of the following duties: vacuuming, cleaning
kitchen and bathroom floors, cleaning bathroom sinks and bathtubs, changing linens and any
other related assistance the older adult may need.
Home Connections is an effort to create a continuum of care and program options for older
adults living independently in Eagle County. Many residents are underserved and are in at-risk
living situations due to the absence of community, non-medical, in-home services, including
housekeeping and home maintenance assistance and non-skilled nursing for bathing and meal
preparation. Our local home health agency supplies skilled nursing services only and there are no
non-skilled nursing, adult day care and respite care agencies in Eagle County. Also, there is no
assisted living facility in Eagle County. Often community residents aged 60 and older are forced
to move out of their communities to receive a higher level of care or they stay at home putting
themselves at risk of accident, injury or illness.
"Lack of adequate services can compromise personal safety and negatively affect the
management of health problems" (AARP Public Policy Institute, 2003) Simple housekeeping is
one of the most difficult tasks for seniors with arthritis, low energy, and limited mobility. Just
making a bed can be exhausting. One current Home Connections client, Georgie, has had
repeated back surgeries in the last year and her husband has been in and out of the hospital and a
nursing facility. She states that Home Connections is helpful for her because "Cleaning is
something that I cannot do. She (the housekeeping proprietor) helps keep the house clean going
often above and beyond to help me with things I cannot do myself"
The lack of in-home services for older adults places an extensive burden on caregivers, family
members and friends, who attempt to care for these older adults in the absence of financial and
programmatic resources. Laura Jean, a daughter and caregiver of a Home Connections client,
completes all the shopping, errand running, and other living necessities for her father while also
working full time outside the county. She is grateful to have another person helping her father
who has limited ability to move due to a stroke. The housekeeping is an essential service
however having the extra eyes on her father while she is working is another source of support.
It is difficult for many individuals and caregivers to afford housekeeping help on their own.
Currently, Home Connections is the only non-medical service in the county other than supports
provided by Eagle County and volunteers.
2. How many Eagle County residents will you serve in this program? What are the
demographics of these clients? Provide historical data if available, and projections for the
future. Please use an unduplicated headcount -- that is, count each individual or family only
once for each year.
In 2010-2011, Home Connections will give housekeeping services to eight adults aged 60 years
and over who are disabled or ill, live at 250% of poverty or below and have few family supports.
During the last year, Home Connections provided housekeeping help to five different older
adults. These individuals range in age from 65 to 87 years and live throughout the county. Three
of the individuals have limited mobility, due to stenosis of the spine, back injuries, and recent
strokes. Two of the clients have limited mobility due to illness. One couple receiving the
assistance, lives in Eagle, the woman has had repeated surgeries on her back and knees, the male
is struggling with a heart condition and has been in and out of the hospital. Another client is a
male, aged 79 years, living in Eagle who has kidney failure and has to travel out of the county an
average of three days a week for dialysis. For each of these individuals, the house cleaning
proprietor vacuums the floors, cleans the bathtub and/or shower, toilet and sink and mops the
floors, which due to mobility problems and poor health, these clients cannot manage on their
own. Home Connections has met an essential need for each of these individuals.
Currently, there are 10 older adults on the waitlist for Home Connections. The older adults live
throughout the county including in El Jebel and Basalt. The individuals range in age from 65 to
86 years old. Many have limited mobility due to broken bones and two are ill. Minimal
marketing has been completed on this program, it is estimated that at minimum there are 20 older
adults in the county in need of the Home Connections services.

4. How will this program be staffed?
Home Connections is a collaborative effort of already existing Aging Services staff. The Home
Connections Coordinator completes program funding grants, organizing contracts for service,
budgeting, communicating and arranging billing and payment with the housekeeping contractor,
and marketing. The Adult Services Case Manager provides case management for older adults
receiving Home Connections services including collecting and assessing new referrals,
completing initial and follow-up assessments to determine who is eligible and supporting with
the coordination of starting the housekeeping in the older adult's homes.
The cleaning services are supplied by Frances Romero, a licensed and insured housekeeping
contractor. The current contractor has experience both volunteering and working with older
adults and enjoys their company and assisting them. This proprietor has been consistently
serving the program for two years. Eagle County pays the contractor $20.00/hr for housekeeping
services as well as an additional $10.00 visit/ client for cleaning supplies.
5. What is the budget for this program? What proportion of the budget do you propose to fund
with United Way contributions? If this is a collaborative project, what is the dollar allocation to
each participating agency?
Eagle County Aging Services is requesting $6000.00 to fund Home Connections Services for
five older adults. The housekeeping services include cleaning two tunes a month for two hours a
session. Eagle County will contribute $3600.00 to enroll three clients to the program as well as
the Home Connections Coordinator time.

6. How will the success of this program/project be quantified, measured and reported? (The
United Way will request results prior to 4`~' quarter allocation of granted funds.)
Eagle County Aging Services will report on the number of clients served using United Way
program funds as well as include information on qualitative data on the older adults who receive
the housekeeping assistance. The successes of the Home Connections program will be measured
throughout the time that clients receive services. Program successes will be quantified by the
number of clients who do not have accidents or injuries and do not require movement into a
higher level of care while receiving the housekeeping services.
7. What are your alternatives for funding and/or program management if the program cannot be
funded to the requested amount?
Eagle County Aging Services plans to budget $3,600 in 2011 for Home Connections however
given the current economic climate it is difficult to say how much will be allocated for this
program. In addition, Eagle County Aging Services will pursue additional grant support. It is
important to say that without the support from United Way of Eagle River Valley, Aging
Services will not be able to provide Hoene Connections to older adults.
8. How will the United Way of Eagle River Valley be recognized as a benefactor of this
program?
In June 2008, Eagle County finalized a Volunteer Services brochure that includes the United
Way logo. Volunteer Services shares the brochure with individuals interested in volunteering
throughout Eagle County as well as multiple organizations. In addition, the brochure is available
to the public in multiple Eagle County offices. Eagle County will recognize the United Way of
the Eagle River Valley as a partner in serving older adults in Eagle County at our Older
Americans Day Celebration at the Gypsum Recreation Center on May 24th. The theme for the
Celebration is Age Strong and Live Long. The Recreation Center is offering free exercise
classes, games and mini seminars on legal issues for older adults. There will be information
booths including Para transit services and RSVP. Our current director is opening the event by
reading a resolution for Older Americans Month and acknowledging our partners including the
United Way of the Eagle River Valley in supporting older Americans in Eagle County.
PART III OVERVIEW of LAST YEAR'S PROGRAM
If UWERV funded or partially funded a program of your agency in 2008-2009, please give an
overview of that program and the clients served. Explain the program's goals, and whether or
not they were met. Are you seeking United Way funds to continue this program? If not, will it
be continued using other funds? Use measurable and clear facts.
The United Way of Eagle River Valley granted Eagle County $2000.00 for Home Connections
in 2009 and Eagle County allotted $3000 for the program in 2010. The goal of Home
Connections is to support local older adults, who are at risk of needing a higher level of care in
assisted living due to illness, injuries or physical disabilities to remain safely and independently
in their own homes. During the last year, Home Connections provided five clients including two
couples and three individuals with bimonthly housekeeping support. One client did decide she no
longer needed the service. Currently, all five clients remain in their home with this continued
housekeeping assistance. Additionally, Home Connections provides a much needed respite for
the caregivers and family members of these individuals so their time can be spent on caring for
other needs including errand running, financial management and self care which are so important
for caregivers. Home Connections is the only free of charge non-medical support program in the
county available to help local older adults with housekeeping and it is vital for senior health and
safety.

NOTE: To qualify as a Community Reinvestment Act (CRA) program, at least 50% of the
participants must be classified as low income to meet the CRA low and moderate income
criteria. CRA-qualified donations allow banks to receive credit from examiners for supporting
their local communities. To facilitate the large donations we receive from banks, the United
Way of Eagle River Valley attempts to document the qualified programs that it supports.
