EAGLE COUNTY AND EAGLE CARE MEDICAL CLINIC OF THE VAIL VALLEY MEDICAL CENTER

Grant Recipient: Eagle Care Medical Clinic of the Vail Valley Medical Center
Grant Amount: $11,050
Grant Period: November 15, 2009 -December 31, 2009
Whereas, Eagle County works to promote the health, safety, and welfare of Eagle County citizens of all
ages; and
Whereas, among the services Eagle County provides in order to promote such health, safety, and
welfare are grants-in-aid to local organizations whose work assists the County in accomplishment of the
goal to create and sustain a safe, healthy, and socially diverse community; and
Whereas, Eagle Care Clinic (hereinafter referred to as "Grant Recipient") has agreed to provide services
that create and sustain a healthy and socially diverse community in Eagle County; and
Whereas, Grant Recipient acknowledges that pursuant to the terms of C.R.S. ยง 24-76.5-101, et seq.,
Eagle County is prohibited from providing certain public benefits to illegal aliens over the age of
eighteen.
Now, therefore, in consideration of the foregoing premises and the following promises, Eagle County
and Grant Recipient enter into this Agreement for payment of the grant award in the amount of Eleven
Thousand and Fifty Dollars (11,050) to Grant Recipient.
A. Grant Recipient agrees that funds awarded by Eagle County may be used and expended solely to
fund quality improvement materials or activities during the Grant Period. In return for the award of the
$11,050 grant, Grant Recipient agrees to submit a report no later than April 15, 2010 including:
(a) The items purchased for exam room refurbishing.
(b) The number of children treated with dental kits.
(c) The number of children provided with fluoride varnishes.
(d) The amount of funding matched by Vail Valley Medical Center.
(e) The number of prenatal and pediatric patients who benefited from the BrightStart
funds.

B. Grant funds under this agreement are to be used and expended within the Grant Period. Any
funds not used or expended during the Grant Period must be returned to Eagle County. Grant recipient
must provide receipts to Eagle County.
C. If, after payment of the Grant Award, Eagle County reasonably determines that payment was
improper because the purposes for which the payment was made were misrepresented, or the terms of
this Agreement were breached, then upon written notice of such determination and request for
reimbursement from Eagle County, Grant Recipient shall forthwith return such payment to Eagle
County.
D. In the event of litigation in connection with this Agreement, the prevailing party shall be entitled
to recover all reasonable costs incurred, including attorney fees, costs, staff time and other claim related
expenses.
E. Grant Recipient shall comply at all times and in all respects with all applicable federal, state and
local laws, resolutions and codes; and specifically, with the requirements of the Civil Rights Act of 1964
and Section 504, Rehabilitation Act of 1973, concerning discrimination on the basis of race, color, sex,
age, religion, political beliefs, national origin or handicap.
F. Grant Recipient shall not assign any of its rights or duties under this Grant Agreement to a third
party without the prior written consent of County. County shall terminate this Grant Agreement in the
event of any assignment without its prior written consent, in which case, any unused Grant Funds must
be returned to County.
G. Grant Recipient agrees that no agent, employee or volunteer of Grant Recipient shall be
deemed an agent, employee or volunteer of County.
H. Grant Recipient shall maintain, for a minimum of three (3) years, adequate financial and
programmatic records for reporting to County on performance of its responsibilities hereunder. Grant
Recipient shall be subject to financial audit by federal, state or county auditors or their designees. Grant
Recipient authorizes County to perform audits or to make inspections during normal business hours,
upon 48 hours notice to Grant Recipient for the purpose of evaluating performance under this Grant
Agreement. Grant Recipient shall cooperate fully with authorized Health and Human Services
representatives in the observation and evaluation of the program and records. Grant Recipient shall
have the right to dispute any claims of misuse of funds and seek an amicable resolution with County.
I. This Agreement shall be governed by the laws of the State of Colorado. Jurisdiction and venue
for any suit, right or cause of action arising under, or in connection with this Agreement shall be
exclusive in Eagle County, Colorado.
J. This Grant Agreement supersedes all previous communications, negotiations, and/or
agreements between the respective parties hereto, either verbal, or written, and the same not expressly
contained herein are hereby withdrawn and annulled.
K. This Grant Agreement does not, and shall not be deemed or construed to confer upon or grant
to any third party or parties any right to claim damages or to bring any suit, action or other proceeding
against either Grant Recipient or County because of any breach hereof or because of any of the terms,
covenants, agreements and conditions contained herein.
L. Invalidity or unenforceability of any provision of this Grant Agreement shall not affect the other
provisions hereof, and this Grant Agreement shall be construed as if such invalid or unenforceable
provision was omitted.
M. Grant recipient agrees to following:
(a) BrightStart Acknowledgement. Contractor shall acknowledge in all public
information materials that the Eagle Care Clinic is a community BrightStart partner.
(b) Network of Care. The Contractor shall cooperate with BrightStart in providing
information to parents, staff, and the public on community early childhood programs
and how to access these early childhood programs for services using the Network of
Care website located at www.networkofcare.org.
(c) Mandatory Reporting. Contractor shall notify HHS immediately of all reports of
suspected child abuse or neglect involving Contractor, including, but not limited to,
employees, volunteers and clients. HHS contractors are considered to be mandatory
reporters for suspected child abuse and neglect and are to make those reports directly
to HHS Children and Family Services - (970) 328-8840.
N. Grant Recipient certifies that it has read the Agreement, understands each and every term and
the requirements set forth herein, and agrees to comply with the same.

Explanation of the proposed services:
The two pediatric exam rooms in Eagle Care Clinic are inadequately furnished with
unsafe and unsightly exam tables, wall-mounted otoscope and opthalmoscope units
which make it difficult to examine squirming lap babies, and one exam stool (broken)
shared between the two rooms. The exam tables have been cited as an infection
control violation due to torn covers and exposed fillings which cannot be disinfected.
We have been offered two basic used tables which are being replaced by Cardiology.
While they are intact, they offer no storage or safety rails. The pediatric providers have
requested 2 pediatric exam tables with adequate storage and safety rails to facilitate
safe examinations, and necessary supplies at hand. They have also requested two
portable otoscope/opthalmoscope diagnostic sets for better access to little ears and
eyes. Finally, they would like to roll around the room freely and not have to carry their
broken stool from one exam room to another.
Our pediatric providers have a frequent need for nebulizers for treatment of respiratory
ailments. The clinic owned five, but three that were loaned out have not been
returned. They ask for two additional units to ensure availability as necessary.
Our obstetric providers are constantly sharing hand-held fetal Dopplers as one of the
three seems always to be in for repair. At least one day per week, prenatal patients are
seen in the Clinic on the Garden Level and in Eagle Care II on Level I, and the care
techs are constantly running up and down the steps to "borrow" the working Doppler.

They have requested two new Obstetric Dopplers to ensure the function and
effectiveness of fetal heart detection.

Needs Assessment: It is our desire to provide high-quality affordable health care in a
safe, clean environment with up-to-date equipment. Our budget does not always
permit replacement of equipment when it can be repaired, taped, or carried instead of
rolled.

Goals and expected outcomes: Safety: Replacement of exam tables will improve the
safety of our little ones with safety rails to keep them from rolling off, and intact
surfaces adequately disinfected.
Effectiveness: Dopplers with the newest technology will
increase the likelihood of early detection of fetal heart tones, rendering unnecessary the
frightening and very expensive ultrasound exams indicated when heart tones escape
detection.
Ease of examination: The longed-for wheeled exam stools
and portable otoscope/opthalmoscope units will facilitate comfortable and thorough
examinations.
Objectives and timeline for accomplishment:
Again, our objective is to provide healthcare in a safe, clean environment with up-to-
date technology. The above items would contribute immediately to our goals.
Target population and/or target area for the program/project:
Eagle Care Medical Clinic serves the uninsured and underinsured population of Eagle
County in our Edwards location.
Number of participants or beneficiaries: Eagle Care has recorded 2937 pediatric
services in the 11 months from 11/4/08-9/4/09.

Community Partnerships:
Eagle Care strives to provide the developmental screening essential to early
identification of children in need of services who receive their immunizations from the
Eagle County Public Health services. In addition, our pediatric providers provide the
most accessible and to some the only accessible sick child care.

Explanation of the proposed services: There are certain critical programs being
initiated in Eagle Care Medical Clinic and elsewhere throughout the County which
cannot be implemented without take-home supplies. By far the largest and most
significant of these is the "Cavity Free by Three" initiative. Eagle Care
practitioners are making a supreme effort to educate prenatal and pediatric
patients and their caretakers on the importance of dental hygiene. Without
supplies to offer at no cost, we are having difficulty implementing the program.
We would like to make the fluoride varnish treatment an integral element of the
well child exam for those patients not being treated through WIC.

Budget Request Narrative: Dental disease is the number one chronic childhood
disease among Colorado children and is routinely seen in early childhood in Eagle
Care Clinic. Here's the news: it is caused by bacteria and most infants are
infected by their parents! Below are some facts, found on the "Cavity Free at
Three" website www.cavityfreeatthree.org
- Dental caries -which are 100% preventable - is the most common
chronic childhood disease, five times as common as asthma; seven times
as common as hay fever.
- 80% of the disease is in 20% of the population. It is largely a disease of
poverty.
- Dental disease affects children's ability to eat properly, grow, attend
school and learn.
- In Colorado 18% of our 2-4 year-olds have dental caries. The infection
rate of the patients in Eagle Care Clinic has not been measured, but it is
significantly higher than the state rate.
Through this grant opportunity, we hope to implement this program at the
Prenatal as well as the pediatric level of care. We want it to become a cultural
imperative for educated parents to stop this disease.

Goals and expected outcomes: The goal is to educate parents, even as early as
in the course of prenatal care, as to the importance of proper dental hygiene and
routine checkups and in turn reduce the pervasiveness of dental disease.
Objectives and timeline for accomplishment: The education and treatment have
been started in Eagle Care Medical Clinic, but slowly because we have been
required to charge for the service and supplies. Patients educated and
scheduled for treatment are failing to show up for their appointments. We
suspect that the up-front cost of the appointment is inhibiting the program.
Target population and/or target area for the program/project: Prenatal and
pediatric patients in Eagle Care Clinic.
Number of participants or beneficiaries: We would like to incorporate this
program into our prenatal and pediatric clinics.
Program strategies/project activities and the use of evidence based practices:
Community Partnerships: In partnership with the WIC program, Eagle Care
hopes to make a significant contribution to reduce the pervasiveness of dental
disease in our population.
